Abstract
There has been a renewed interest in neurocognitive functions in schizophrenics, recognizing their importance for rehabilitation of schizophrenics. Second generation antipsychotics (SGAs) are reported to be superior to first generation antipsychotics (FGAs) in it's effect on neurocognitive functions in schizophrenics. This paper reviewed studies on effects of antipsychotics on neurocognitive functions in schizophrenics.1) Low dose treatment of FGAs, in limited degree, improved neurocognitve functions in schizophrenics, such as concentration, sensory perceptual function, verbal function, and executive function, but not memory function. 2) SGAs are superior to FGAs in overall effectiveness on neurocognitive functions in schizophrenics. Clozapine improved concentration, working memory, verbal memory, verbal fluency, and visual motor function. Risperidone improved concentration, executive function, working memory, and verbal memory. Olanzapine improved concentration, verbal fluency, verbal memory, and executive function. Quetiapine improved concentration, verbal fluency, verbal memory, and motor function. Ziprasidone improved concentration, immediate recall, and delayed recall.3) There appears to be some differential effects among SGAs on neurocongnitive domains, however, further studies are in need to draw a definitive conclusion. 4) Although the pathophysiology of neurocognitive impairments in schizophrenia is not well elucidated yet, SGAs are thought to improve the neurocognitive functions via enhancing dopamine and acetylcholine functions in prefrontal cortex.
