방문보건서비스의 효율적 운영을 위한 방문경로 최적화 연구

초록

최근 공공보건기관을 중심으로 운영되는 방문보건서비스에 대한 수요가 꾸준히 늘고 있다. 방문보건서비스는 특히 의료시설에 대한 접근성이 낮고, 거동이 불편한 노년인구가 많은 농촌지역에 효과적인 서비스로 인식되고 있다. 수요와 서비스 효과에 대한 긍정적 인식에도 불구하고, 현재 농촌지역의 방문보건서비스를 수행하기 위한 의료자원은 충분하지 못한 실정이다. 본 연구에서는 현재 실시되고 있는 방문보건서비스의 운영현황을 파악하고, 의료 형평성 개선 및 서비스의 효율적 운영을 위한 방문경로 최적화 방안을 모색하고자 한다. 이를 위해 방문보건서비스 경로의 총 이동거리를 최소화하는 최적화 모델을 수립하였고, GIS와 최적화 기법을 이용하여 공간적 대안을 제시하였다. 충청북도 괴산군을 대상으로 사례연구를 수행했으며, 최적화된 방문경로 및 일정을 제시하였다. 본 연구를 통하여 제시된 최적화 모델은 방문빈도, 대상자별 서비스 수요 시간, 방문보건 인력의 업무균형 등 다양한 측면에서 방문보건서비스 운영의 효율성을 향상시키는 대안을 제공할 수 있음을 보여준다.

Abstract

Visiting health care services by the public sector are becoming popular, and their demand is continuously increasing because they can effectively provide nursing or other health care services to rural areas with a lack of accessibility to the services and a high rate of elderly having low mobility. Visiting health care services, however, have not been delivered in effective and efficient ways to the rural elderly due to limited health resources. In this paper, the visiting health care systems currently in operation are examine. Then, the ways of optimizing routing for visiting health care services are explored by focusing on management efficiency and health equity. For doing this, we introduce an optimization model minimizing the total travel distance of visiting routes and solve the problem by utilizing GIS(Geographic Information Systems) and optimization techniques. As an application, we present optimal routes and schedules for visiting health care services in Goesan-Gun, Chungcheongbuk-Do. The results show that the suggested optimal routes could be alternatives in reality with respect to the improvement of visiting counts per day, the equitable provision of service time, and the balance of workload among visiting staffs.
