안와 상부에 위치한 구후부의 안와 해면혈관종- 상안와를 통한 제거 1예

목적: 안와 상부에 위치한 큰 구후부 해면혈관종을 신경외과와 협진하 눈썹위 절개 및 상부안와벽제거를 통한 상부안와 개두술로 완전절제 하였기에 이를 보고하고자 한다. 증례요약: 31세 여자환자가 내원 약 1개월 전 시작된 좌안의 안구돌출을 주소로 내원하였다. 양안의 시력 및 안압은 정상이었고 안구돌출계 검사상 우안 15 mm, 좌안 18 mm였으며 안구운동 장애나 복시는 없었다. 컴퓨터 단층 촬영 및 자기공명영상 검사상 상부 구후부에 상직근, 안검거근을 밀고 있는 크기 2.0×2.6×2.0 cm의 큰 종괴가 관찰되었다. 6개월 후 신경외과에서 상부안와 개두술을 시행하였으나 종양의 접근에 실패하였고, 14개월 후 종양이 다시 증가된 소견 보여 안과와 신경외과와 협진하 눈썹위 절개 및 상부안와벽제거를 통한 개두술로 종양 완전절제술에 성공하였으며 병리검사상 해면혈관종으로 확인되었다. 술 후 안구돌출소견은 호전되었고정상 시력을 보였으나 안구의 상전장애 및 안검하수가 남았고 술 후 6개월째 점차 호전되는 양상을 보이고 있다. 결론: 안와 상부에 위치한 크기가 큰 구후부의 해면혈관종을 상부 안와벽제거를 통해 성공적으로 제거하였음을 보고하는 바이다.
Abstract
Purpose: To report a complete excision of a large superior orbital retrobulbar cavernous hemangioma using supraorbital craniotomy through the suprabrow approach combined with superior orbital wall removal. Case summary: A 31-year-old woman presented to our clinic with left exophthalmos beginning one month earlier. Visual acuities and intraocular pressures of both eyes were normal and there was no diplopia and no limitation in ocular movement. On exophthalmometry,the right eye was 15 mm and the left eye was 18 mm. CT and MRI demonstrated a superior orbital retrobulbar mass of 2.0×2.6×2.0 cm in size, and the mass displaced the superior rectus and levator palpebrae muscles. After six months, a neurosurgeon attempted a classic supraorbital craniotomy but failed. Fourteen months later, the authors completely excised the tumor through superior orbital wall removal. The tumor was confirmed as a cavernous hemangioma after histopathologic study. Conclusions: The authors succeeded in complete excision of a superior orbital retrobulbar cavernous hemangioma using supraorbital craniotomy through a suprabrow approach combined with superior orbital wall removal.
