복시를 주소로 내원한 에드하임-체스터병 1예

목적: 복시를 주소로 내원한 에드하임-체스터병(Erdheim-Chester disease) 1예를 경험하였기에 보고하고자 한다. 증례요약: 56세 여자가 6주 동안 좌측 주시 때 복시가 있어 내원하였다. 우안의 경미한 내전장애를 보이며, 험프리 자동시야검사에서아래쪽 양이측사분맹이 보였다. 안와 및 뇌 자기공명영상에서 양측 안와의 근원추 내에 균일하게 조영증강되는 둥근 종괴가 관찰되었고, 뇌에서도 균일한 신호강도를 보이는 다수의 종괴가 관찰되었다. 전신 검사 상 심장, 폐는 침범되지 않았으나 장골과 후복막을 침범한 소견이 관찰되었다. 우측 안와 종괴에 대한 절개생검을 시행한 결과 지방을 함유한 다수의 조직구와 소수의 Touton 다핵거대세포가관찰되었고, 면역조직화학검사에서 CD68에는 양성이나 CD1a에는 음성을 보여 에드하임-체스터병으로 진단하였다. 방사선 치료 및인터페론-α 치료를 시행하였으나, 2개월 후 요로감염으로 인한 패혈증으로 사망하였다. 결론: 안구돌출 증상 없이 복시가 안와 종괴의 유일한 초기 증상일 수도 있다. 복시가 있는 양측성 안와 후부의 종괴 환자에서 드물지만에드하임-체스터병을 감별진단에 포함해야 할 것이다.
Abstract
Purpose: We present a case of Erdheim-Chester disease (ECD) with diplopia. Case summary: A 56-year-old woman came to the hospital with a 6-week history of diplopia on left lateral gaze. The righteye showed mildly limited adduction. Humphrey automated perimetry demonstrated inferior bitemporal quadrantanopia. Orbital and brain magnetic resonance imaging revealed well-defined orbital masses in both intraconal orbits with homogenousenhancement, as well as multiple masses of homogenous signal intensity in the brain. Systemic evaluation showedinvolvement of the long bones, and retroperitoneum, but no involvement of the heart, or lungs. Incisional biopsy of the rightorbital mass was performed. Histopathological examination showed numerous lipid-laden histiocytes and few multinucleatedTouton giant cells. Immunohistochemical staining showed positivity for CD68, but negativity for CD1a, and ECDwas therefore diagnosed. The patient received treatment with radiation therapy and interferon-α, but died due to sepsissecondary to urinary tract infection after 2 months. Conclusions: Except exophthalmos, diplopia may be the only initial symptom of an orbital mass. Although rare, the possibilityof ECD should be considered in the differential diagnosis of both retrobulbar and orbital masses with diplopia.
