특발성 진성 수정체낭 분리 환자에서의 백내장 수술 1예

목적: 현재까지 알려진 유발인자에 노출된 바 없는 특발성 진성 수정체낭 분리 환자에서 성공적인 백내장 수술 시행 후 주사전자현미경(scanning electron microscope, SEM) 및 투과전자현미경(transmission electron microscope, TEM)으로 확진하여 이를 보고하고자한다. 증례요약: 협심증과 고혈압의 과거력을 가진 77세의 남자 환자가 1년간의 우안 시력저하를 주소로 내원하였다. 세극등 검사상 양안에핵경화성 백내장이 관찰되었고, 우안에는 전방 내에 떠다니는 고리 모양의 섬유성 막이 관찰되어 백내장을 동반한 진성 수정체낭분리로 진단하였다. 전방 내 떠다니는 섬유성 막의 경계보다 조금 더 크게 수정체낭 원형 절개술을 시행하여 별다른 합병증 없이우안의 백내장 초음파 유화술 및 후방 인공수정체 삽입술을 시행하였다. 수술 6개월 후 우안 나안 시력은 20/20, 안압은 18 mmHg로양호한 경과를 보였다.
Abstract
Purpose: We present a case of uneventful cataract surgery in an idiopathic true exfoliation patient with areas of capsular delaminationbased on scanning electron microscope and transmission electron microscope results. Case summary: A 77-year-old male presented with gradual deterioration of vision over 1 year in duration. Slit lamp examinationrevealed bilateral nuclear sclerotic cataracts with ring-shaped fibrous membrane floating within the anterior chamber in the righteye. In addition, the patient was diagnosed with cataract and true exfoliation of the right eye. He underwent uneventful phacoemulsificationand posterior chamber intraocular lens implantation by placing capsulorrhexis outside the delaminated capsulemargin. At 6 months after cataract surgery, the patient showed favorable visual outcome with uncorrected vision of 20/20 and intraocularpressure of 18 mm Hg in the right eye.
