특발성 시신경주위염의 임상적 특징과 치료결과

목적: 특발성 시신경주위염 환자의 임상양상 및 치료경과를 조사하여 이 질환을 진단하고 치료하는 데 도움이 되고자 하였다. 대상과 방법: 본원에 내원한 환자 중 특발성 시신경주위염으로 진단받은 10명 13안의 의무기록을 후향적으로 조사하였다. 결과: 시신경주위염환자 10명의 평균연령은 56.9 ± 9.3세(35세-77세)였고 남녀성비는 5:5로 동일하였다. 주증상으로는 9명 12안(92%)에서 안구통을 동반한 시력저하를 호소하였다. 초진 시 시력 중앙값은 0.2 (안전수지-0.8)였으며, 상대구심성동공운동장애는12안에서 관찰되었다. 한편, 동반된 안와 질환으로 후공막염 1안, 외직근염 1안이 있었다. 안저검사상 5명 8안(61.5%)에서 시신경유두부종 소견을 보였으며 자기공명영상검사상 모든 시신경주위염환자에서 시신경수초와 주위 연부조직의 조영 증가 소견이 보였다. 경구 스테로이드 혹은 고용량 스테로이드 정맥 주사 치료 후 모든 환자에서 시력호전을 보였으며, 이 중 4명(40%)에서 스테로이드 감량중 재발하여 경구 스테로이드로 재치료하였다. 결론: 본 연구에서 시신경주위염은 50대 이상의 장년층에서 안구통을 동반한 급격한 시력저하를 특징으로 하였으며, 자기공명영상검사에서 시신경수초에 국한된 조영 증강을 관찰할 수 있었다. 스테로이드치료에 양호한 반응을 보였으나, 비교적 재발의 빈도가 높아스테로이드 감량 시 재발에 주의할 필요가 있었다. 이러한 임상적, 방사선학적 특징과 치료결과를 통해 이 질환을 진단하고 치료하는데 도움이 될 것으로 생각한다.
Abstract
Purpose: To investigate the clinical and radiologic manifestations of idiopathic optic perineuritis (OPN), and to evaluate the outcomesof steroid treatment for OPN. Methods: We reviewed the medical records and radiologic findings of 10 patients (13 eyes) who were diagnosed with OPN andtreated with steroid. Results: The mean age was 56.5 ± 9.3 years (range, 35-77 years) and the sex ratio was equal. The main complaint was decreasein visual acuity combined with ocular pain during extraocular eye movement in 9 patients. The median visual acuity at thefirst visit was 0.2 (HM-0.8) and the relative afferent papillary defect was observed in 12 eyes. Additionally, combined orbital diseasesincluded posterior scleritis in 1 eye and myositis in 1 eye. Orbit magnetic resonance imaging (MRI) scans demonstratedintraorbital optic nerve sheath enhancement in all patients, occasionally with orbital fat involvement. All patients demonstratedimproved visual acuity after high-dose oral steroid therapy (6 patients) or intravenous (IV) pulse steroid therapy (4 patients). Relapse occurred in 4 patients during steroid tapering. Conclusions: The population in this study was composed predominantly of patients with OPN in their 50’s. The primary symptomof OPN was visual acuity decrease combined with ocular pain during extraocular eye movement. Radiologically, orbit MRI scansdemonstrated intraorbital optic nerve sheath enhancement. The patients in this study demonstrated good responses to steroidtreatment, but clinicians must be aware of the high recurrence rate during steroid tapering in this condition. A combination of clinicaland radiologic findings was helpful to diagnose OPN.
