각막 문신술 후 발생한 알터나리아 균종 감염 1예

초록

목적: 각막 문신술 후 발생한 Alternaria 균종 감염으로 인한 각막 천공을 전층 각막 이식술로 성공적으로 치료한 1예를 경험하여 이를보고하고자 한다. 증례요약: 65세 남자가 좌안 각막 혼탁으로 각막 문신술을 시행하였다. 1달 뒤 좌안 각막 상피 결손 및 괴사 소견 보여 시행한 도말배양 검사상 Alternaria 균종이 배양되었고 0.3% 암포테리신 B 조제안약, 이트라코나졸 100 mg 경구 투여 치료하였으나 3주 뒤 좌안각막 천공 소견 보여 좌안 전층 각막 이식술 시행하였다. 수술 후 안압 상승되어 좌안 섬유주 절제술 시행하였고 2개월간 경과관찰에서 이식된 각막은 잘 유지되어 있었으며 각막염의 재발 소견은 보이지 않았다. 결론: 각막 문신은 상피 및 앞쪽 실질의 손상과 스테로이드 사용으로 인한 감염의 위험 요인이 증가하게 되고, 감염 시 일반 각막에비해 감염 여부 및 원인균 감별이 쉽지 않다. 따라서 수술 중 또는 수술 후 감염에 주의하고 자세히 관찰을 해야 할 것이다.

Abstract

Purpose: To report a case of successful treatment of Alternaria species infection after corneal tattooing using penetrating keratoplastycorneal perforation. Case summary: A 65-year-old male underwent corneal tattooing for corneal opacity of the left eye. One month later, epithelial defectand necrosis of the left eye were observed, thus a smear and culture were performed and Alternaria species were cultured. He was treated with 0.3% amphotericin B-fortified eyedrops and Itraconazole oral medication; however, 3 weeks later, cornealperforation of the left eye was observed and penetrating keratoplasty was performed. Postoperatively, due to persistent increasedintraocular pressure, trabeculectomy was performed. Two months later, donor cornea was well-grafted and recurrentkeratitis was not observed. Conclusions: Corneal tattooing can cause damage to the corneal epithelium and anterior stroma and increase the risk of infectionwhen using steroids. In the case of infection, detecting and differentiating the type of microorganisms are more difficult. Therefore, proper care and close monitoring for the signs of infection are important during and after the surgery.
