청소년의 위험 성행동과 건강결과 간의 관계: 생태체계요인과 성별차이를 중심으로

Relationship between Risky Sexual Behaviors and Health Outcomes among Adolescents: Focused on Ecological Factors and Gender Discrepancy

초록

이 연구는 건강위험행동의 맥락에서 청소년의 위험 성행동과 건강결과 간의 관계를 생태체계요인과 성별차이를 중심으로 확인하고자 하였다. 궁극적으로 개인 속성을 넘어 사회제도⋅문화⋅환경 등의 영향을 고려해 청소년의 건강결과에 미치는 부정적 영향을 완화할 수 있는 사회적 개입지점의 정책과 제도를 모색하고자 했다. 분석자료는 제12차 청소년건강행태조사(2016년)를 활용했다. 복합표본분석을 적용하여 다중로지스틱 회귀분석, 경로분석, 다중집단분석을 실시했다. 생태체계요인이 위험 성행동에 미치는 영향을 다중로지스틱 회귀분석을 통해 살펴본 결과, 개인 수준에서는 성별, 용돈, 신체활동, 음주, 약물복용경험, 학업성적, 아르바이트 경험이 통계적으로 유의했다. 친구수준에서는 건강위험행동을 하는 친구 유무가, 가정수준에서는 가정경제 수준이, 학교수준에서는 학교유형이, 지역수준에서는 지역규모가 통계적으로 유의한 결과를 보였다. 생태체계요인을 통제하고 경로분석을 시행한 결과, 위험 성행동은 성건강(성병감염경험), 신체건강(구강건강, 폭력손상), 정신건강(스트레스, 우울, 자살생각)과의 관계가 통계적으로 유의하였다. 또한, 다중집단분석 결과, 위험 성행동이 건강결과에 미치는 영향에서 성별간의 차이를 확인할 수 있었다. 이 연구는 청소년의 위험 성행동을 일탈이나 비행이 아니라 건강위험행동의 관점에서 다루었다는 데 의의가 있다. 또한, 브론펜브레너의 생태체계모형에 따라 개인수준을 넘어 청소년을 둘러싼 사회적 관계와 맥락적 요인까지 고려했을 뿐 아니라, 생태체계요인을 통제한 후에 건강결과지표와의 관계를 확인하고, 남녀집단의 차이까지 확인했다는 데 의의가 있다.

Abstract

This study aimed to investigate in the context of risky health behaviors the relationship between risky sexual behaviors and health outcomes among adolescents focused on ecological factors and gender discrepancy. With social institutional, cultural, and environmental factors as well as individual factors into consideration, we intended to find social policy intervention measures that could alleviate the negative influences of adolescents’ risky sexual behaviors on health outcomes. A secondary data analysis method was employed in this study. Using data from the 12th wave of the Korea Youth Risk Behavior Web-based Survey of year 2016, a series of logistic regression, path analysis, and multi-group analysis were conducted. Results from logistic regression investigating the relationship between ecological factors and risky sexual behaviors showed that individual level factors including gender, pin money, physical activity, alcohol drinking behavior, drug use, academic performance, and job experience were significant predictors of adolescents’ risky sexual behaviors. Whether having peers conducting risky health behaviors in peer level, financial status in family level, type of school in school level, and the size of the cities in neighborhood level were significantly associated with adolescents’ risky sexual behaviors. Results from the path analysis after controlling for ecological factors showed that risky sexual behaviors were associated with sexual health (sexually transmitted disease), physical health (oral health, damage from violence), and psychological health (stress, depression, suicidal ideation). Also, the multiple group analysis results indicated the gender discrepancy in the relationship between risky sexual behaviors and health outcomes. This study contributes to the existing literature by taking adolescents’ risky sexual behaviors not in terms of juvenile delinquency or deviation but in terms of risky health behaviors. Also, it is important to note that this study following Bronfenbrenner’s ecological system model take into account not only individual factors but also social relationship and contextual factors. Further, this study found the relationship between risky sexual behaviors and health outcomes among adolescents, and the gender discrepancy in the relationship.
