청소년 건강행태조사: 중학교 3학년을 중심으로

초록

본 연구는 중학교 3학년 청소년들의 주요 건강행태율을 포괄적으로 측정하였고, 흡연 및 음주와 다른 건강위험행태와의 관련성 및 청소년들의 건강행태에 미치는 사회적 영향을 조사하고자 실시하였다. 수원시 및 진주시 2개 지역에서 지역별 4개 중학교 3학년 남·여학생 3,739 학생들이 조사에 참여하였다. 본 연구의 조사대상 건강행태는 흡연 및 음주, 불법약물을 포함한 기타약물사용 경험, 조기 성경험, 우울 및 자살관련 경험 등의 정서적 위험행태, 반사회적 행동, 학교문제, 폭력 및 왕따관련 행태, 도박성 게임 경험 등의 주요 건강위험행태와 안전벨트 착용, 체중인식 및 체중조절 등의 건강증진행태 등이었다. 또한 청소년들의 건강위험행태에 미치는 사회적 영향을 측정하기 위해 부모, 형제, 친구, 주위 어른들의 건강위험행태 여부를 조사하였다. 자료 분석은 빈도분석 및 교차분석과 단순 로지스틱 회귀분석 등을 활용해서 교차비(Odds Ratio) 등을 산출했다. 연구결과, 조사 참여 청소년들의 건강위험행태율이 전반적으로 다른 선행연구 결과보다 높게 나타났다. 반면, 건강증진행태율 수준은 아직 낮았다. 성별 차이를 보면, 남학생은 흡연 및 음주, 폭력관련 행태, 조기 성경험과 도박성 게임 경험률이 여학생보다 유의하게 높게 나타난 반면, 여학생은 우울 및 자살시도 등 정서적 위험행태 및 왕따 피해 등에 있어서 남학생보다 유의하게 높게 나타났다. 양부모 생존여부에 따라서는 대부분의 청소년 건강행태율에서 차이가 있었는데 특히 여학생에서 유의했다. 흡연 및 음주와 다른 건강위험행태와의 상호관련성은 거의 모든 건강위험행태에서 유의하게 나타났고, 특히 흡연 및 음주를 모두 하는 경우는 높은 상호관련성을 보였다. 한편, 가족 및 주위의 건강위험행태가 청소년 건강위험행태에 미치는 사회적 영향 측정결과, 친구 등 또래집단의 건강위험행태가 가장 많은 영향을 주는 것으로 나타났으며, 그 다음 형제, 부모, 주위 어른 순이었다. 그러므로 이러한 본 연구결과를 통해서 볼 때, 청소년 건강위험행태 예방을 위한 효과적인 접근을 위해서는 첫째, 청소년 건강위험행태를 지속적으로 관찰할 수 있는 국가수준의 청소년 건강행태감시체계 수립과 표준화된 조사도구 및 조사실시방법 등에 관한 지침이 마련되어야 하고, 둘째, 청소년 건강위험행태에 대해 행태별 개입보다는 건강행태 전반에 대해 통합적인 개입이 필요하며, 셋째, 이와 함께 사회영향에 대처할 수 있는 사회기술 습득 등을 포함한 다 차원 및 다 수준의 포괄적이고 통합적인 개입이 필요하다고 생각한다. 한편, 본 연구의 결과는 우리 청소년들의 주요 건강위험행태 실태에 관한 포괄적인 기초 자료를 제공할 뿐만 아니라, 향후 효과적인 건강위험행태 예방 프로그램 및 정책 개발을 위한 중요한 기초 자료로서 활용할 수 있을 것으로 기대한다.

Abstract

Objectives: Our objective in this study was to measure the frequency of health- related behaviors amongst ninth grade students in Korea. The study investigated the association of alcohol and tobacco use with other health risk behaviors. We also examined social influences that affected youth health behaviors. Methods: A survey questionnaire was administered to 3,739 ninth grade students across eight schools in two cities in Korea. The questionnaire measured the frequency of tobacco, alcohol, and substance use, sexual intercourse, anti-social behaviors, violence, feelings of social isolation at school, depression or attempted suicide, absenteeism from school, gambling, weight control, and seatbelt use. The survey accounted for the social influence of parents, siblings, peers, and other adults on students' health behaviors. Data was analyzed with frequency, chi-square test, and logistic regression. Results: Students reported higher frequency of most of health risk behaviors than is reported in other studies on youth health behaviors in Korea. Male students reported statistically higher frequency than female students of tobacco and alcohol use, violence, sexual intercourse, and gambling; female students reported statistically higher frequency than males of depression or attempted suicide, and feelings of social isolation at school. The absence of one parent or both parents affected students' health behaviors, particularly among female students. The tobacco and alcohol use were statistically strongly associated with most of other health risk behaviors among students. Compared with parents, siblings, and other adults, peers' influence on students' tobacco and alcohol use were much stronger. Conclusions: Korea youth health-related behaviors should be monitored consistently in a surveillance system at the national level, and be improved through multi- dimensional and multi-level health promotion efforts in Korea.
