소아에서 3급 부정교합의 진단과 처치

전치부 반대교합은 3급 부정교합의 가장 큰 trait이다. 다른 부정교합의 유형과는 달리 전치부 반대교합은 치료가 가장 dramatic하게 이루어지므로 치과의사들이 쉽게 접근하여 치료를 시행할 수 있는 분야라고 생각될 수 있으나 실제 치료 후 장기적인 예후를 관찰해 보면 예후가 별반 좋지 않거나 문제 발생의 소지가 많은 부정교합 유형중에 하나이다. 치아 몇 개만 전후로 이동시켜 교합을 jumping시키면 반대교합이 해결될 것같아 쉽게 달려들었다가 예상치 못한 어려운 문제들이 하나 둘 표출될 때 당황하게 되는 경우가 종종 있다. 전치부 반대교합은 보기에는 간단히 보여도 그 이면에 내재되어 있는 문제가 많은, 흡사 빙산의 일각 같은 증례들이 많다. 그러나 한편으로 증례에 따라서는 간단하고 dramatic하게 치료되므로써 환자나 보호자에게 신뢰감을 쌓을 수 있는 유형도 있다. 따라서 3급 부정교합은 잠재되어 있는 문제점을 파악할 수 있는 진단학적 안목을 가지는 것이 무엇보다도 중요하며 이와같은 진단학적 안목을 바탕으로 선별하여 치료를 하는 것이 3급 부정교합의 치료에 접근하는 바람직한 방법이다.
