가정간호 프로그램이 미숙아 어머니의 불안, 모성자신감 및 미숙아의 성장, 의료이용율에 미치는 영향

초록

미숙아 출산이 늘면서 미숙아 어머니의 어려움을 이해하고 미숙아와 미숙아 어머니를 돕고자 하는 여러 가지 중재방안이 개발되고 그 효과를 보기 위한 시도는 있었으나 미숙아가 중환자실에서 퇴원하는 초기에 적극적으로 이루어지는 가정방문과 관련된 중재 효과를 검증한 국내 연구는 없었다. 미숙아는 다른 환자들과 달리 중환자실에서 바로 집으로 퇴원을 하게 된다. 중환자실에 있는 동안 보호자는많은 퇴원교육을 받게 되지만 미숙아의 가정은 신생아중환자실과 전혀 다른 환경으로 병원의 의료진은 각각의 가정환경과 가족상황에 따라 보호자가 어렵지 않게이행할 수 있는 최적의 교육을 시행하기에는 한계가 있다. 그러므로 미숙아 어머니들이 퇴원 초기에 의료진의감독 없이 혼자 미숙아를 돌보고 특수간호를 시행하며아기의 상태관찰을 하는 것은 많은 부담과 어려움이 따를 수 밖에 없다. 이런 상황에서 미숙아와 미숙아 어머니가 계속해서 지낼 가정으로 직접 방문하여 그들에게제공하는 지지와 격려는 미숙아 어머니에게 긍정적인힘으로 작용할 것이다. 의료기관 가정간호서비스는 아기가 퇴원한 병원의가정전문간호사가 방문하게 되므로 퇴원초기에 발생할수 있는 여러 가지 문제점을 병원의 의료진과 신속히의사소통하며 해결할 수 있는 장점이 있다. 본 연구에서는 가정간호 프로그램이 미숙아의 성장, 의료기관 서비스 이용에 긍정적인 영향을 미치는 지는 확인하지 못했으나 프로그램의 기간, 내용, 가정방문 횟수를 조정하여 추후 연구를 통하여 확인할 필요가 있겠다. 미숙아는 퇴원 초기의 신체적 불안정을 이겨내더라도 계속 성장하면서 섭식의 어려움, 발달의 어려움, 여러 가지 장애 노출의 가능성이 있어 퇴원 후 의료기관가정간호서비스 외에도 지속적인 사정과 중재가 필요하다. 현재 우리나라는 미숙아 사업으로 의료비 지원 등다양한 미숙아 사업이 시행되고 있으나 미숙아들이 건강하게 성장하여 국가 원동력으로 활동할 수 있도록 지속적인 지역사회와 국가의 관심과 지원이 필요하겠다. 본 연구의 결과를 바탕으로 다음과 같은 제언을 하고자 한다. 첫째, 가정간호 프로그램의 기간, 내용, 가정방문 횟수를 조정하여 미숙아 대상 가정간호 프로그램의 효과에 대한 반복연구를 제언한다. 둘째, 의료기관 중심 가정간호서비스와 지역사회의방문건강관리서비스를 연계한 단기, 중기, 장기의 미숙아 관리사업의 개발 필요성을 제언한다. 셋째, 미숙아 대상 가정간호서비스를 전국으로 확대하기 위해서는 가정전문간호사를 위한 전문적인 교육과 훈련 프로그램을 개발하는 것이 필요하다. 넷째, 가정간호 프로그램이 재원일수 단축, 의료비용절감 등 다른 변인에도 효과가 있는지 검증하는 추후 연구가 필요하다.

Abstract

Purpose: The purpose of this study was to develop the home health care program (HHCP) for the premature infant and their mother and to investigate its effects on the premature infant's growth, readmission, emergency visits, unplanned hospital visits, the anxiety of the premature infant's mother and the confidence of maternal role. Methods: This study was the nonequivalent control pretest-posttest. The 43 premature infants and their mothers participated in this study. The controlled groups were 22 and the experimental groups were 21. The HHCP developed though the researched review and analysis of the premature infant's mother’s requests. Results: The HHCP was composed of preparing discharge, premature infant care, and telephone counseling. Premature infant care was composed of feeding the baby safely, using medical device, assessing baby, bathing and touching. The HHCP had effects on premature mother's anxiety (p= 0.04), maternal role’s confidence (p=0.001) and had no effects on the premature infant’s growth, readmission, emergency visits, unplanned hospital visits between two groups. Conclusion: The HHCP was effective on the premature infant mother's anxiety and maternal role’s confidence. Anxiety reduced, and maternal role’s confidence increased. To help the premature infant and their mother and to settle down, Home Health Nursing service had to be arranged before the discharge.
