99mTc-MIBI parathyroid scans are useful for visualizing functionally hyperactive lesions; however, their reliance on visual qualitative evaluation entails limitations such as subjectivity, inter-observer variability, and decreased sensitivity. This study evaluated the clinical utility of quantitative analysis using the parathyroid Uptake Ratio. From January to August 2025, 20 patients who underwent 99mTc-MIBI parathyroid scans at the Department of Nuclear Medicine, ‘S’ Hospital, were included. After preparation of the radiopharmaceutical and intravenous injection, early-phase images (15 minutes post-injection) and de- layed-phase images (2 hours and 30 minutes post-injection) were acquired using a dual-head gamma camera. Through an im- age analysis program, the parathyroid glands were divided into right and left, upper and lower lobes, and regions of interest were set for each to measure counts. Uptake Ratio was calculated after background correction. Statistical analysis was per- formed using Two-Way ANOVA (SPSS ver. 29). Cases with an Uptake Ratio of 1.5 or higher were identified in 9 patients, which completely corresponded with clinical interpretations such as parathyroid hyperplasia, adenoma, and mildly delayed MIBI retention. The results of quantitative analysis showed a high concordance with clinical interpretation, reducing the possibility of false negatives and improving diagnostic accuracy. Therefore, quantitative analysis using Uptake Ratio provides an objective and reproducible evaluation, and may serve as a useful adjunct tool for accurate diagnosis of parathyroid diseases.
Abstract
