요양병원과 요양시설 노인의 건강상태, 사회적 지지 및 거주만족도

본 연구는 요양병원과 요양시설 노인들의 일반적 특성과 건강관련 특성 및 일상생활수행능력 정도를 파악하고 건강문제, 사회적 지지 정도, 거주만족도를 비교, 분석한 연구이다. 그 결과 요양병원 입원 노인과 요양시설 입소 노인들의 입원 동기는 유의하게 달랐으나 건강지표는 유의한 차이가 없었으며 요양병원에 1년 이상 입원하고 있는 노인이 71.7%나 되어 요양병원과 요양시설이 혼재되어 사용되고 있음이 확인되었다. 사회적 지지 정도는 구조적 속성에서 요양병원이 요양시설보다 시설 직원, 시설 외부인과의 접촉 빈도가 더 많았고, 요양시설이 요양병원보다 시설 내 동료와의 접촉빈도가 더 높게 나타났다. 그러나 정서적, 도구적 지지를 받는 기능적 속성에서는 요양시설의 노인이 요양병원 노인보다 시설 내 동료와 시설 직원에게 더 유의하게 더 많은 지지를 받고 있었으며, 거주만족도도 요양병원보다 요양시설 거주 노인에게서 유의하게 더 높은 것으로 나타났다. 본 연구는 요양병원과 요양시설의 기능 재정립을 위한 제도 마련과 서비스 질 향상을 위한 기초적 자료를 제공하였다는 데에 의의가 있으나, 사회적 지지의 구조적 속성과 기능적 속성의 정서적지지 도구의 신뢰도가 낮고, 일부 요양병원과 요양시설만을 임의 표출하여 조사한 결과이며, 인지기능이 저하되어 있는 치매 노인이 포함되어 있지 않아 일반화하는 데에 제한점이 있으므로 이를 보완한 반복 연구의 필요성을 제언한다.
Abstract
Purpose: The purpose of this study was to identify and compare the differences on perceived health status, Activities of Daily Livings(ADL), social support, and residential satisfaction between long-term care hospitals and nursing homes. Methods: Data was collected through questionnaires and interviews conducted from March 29 to April 22, 2011. The subjects were 66 old adults in 3 long-term care hospitals and 53 old adults in 6 nursing homes. Data were analyzed by Pearson's correlation analysis and t-tests. Results: ADL and subjective health status in nursing homes were worse than those in long-term hospitals, but it was not statistically significant (p>.05). Old adults in nursing homes received more emotional support from other residents and staff, and received more instrumental support from staff than those in long-term care hospitals (p<.001). The mean scores of resident satisfaction in long-term care hospitals and nursing homes were 3.53(SD=±0.36) and 3.97(SD=±0.44), respectively. Resident satisfaction in nursing homes significantly was higher than long-term care hospitals (p<.001). Conclusion: Health care personnels in long-term care hospitals should enhance resident satisfaction and social support and need to coordinate long-stay patients with nursing homes.
