정신증적 장애의 회복 관점에 기초하여 수용전념치료가 적용된 연구의 체계적 문헌고찰 및 메타분석

초록

수용전념치료(ACT)는 범진단적 접근으로서 정신증의 회복에 대해 효과가 검증되었으며 갈수록 널리 활용되고 있으나 국내에서는 관련 연구가 거의 없다. 본 연구는 정신증적 장애가 있는 참여자들을 대상으로 2002년부터 2021년까지 수용전념치료를 적용하여 그 효과성을 검증한 문헌들을 체계적으로 고찰하고자 하였다. 따라서 수용전념치료가 시행된 문헌들 12편의 질적인 특성들에 대해 우선 검토한 후, 양적 분석을 위해 메타분석을 시행하였고 8편의 문헌들에서 도출한 치료의 효과 크기를 산출하였다. 또한 그 후 문헌들의 결과 측정치에 대해 임상적 회복 및 개인적 회복에 대한 효과성에 대해 각각 세부적으로 검토해 보았다. 연구 결과, 정신증 증상을 경험하는 참여자 집단에서 수용전념치료를 적용한 대다수의 문헌들은 임상적 회복인 양성 증상에 대해서 유의한 효과를 나타내었을 뿐만 아니라, 개인적 회복 중 음성증상 및 주관적 고통감에서 역시 유의한 효과를 나타내었다. 본 연구 결과 정신증적 장애가 있는 당사자의 개인적 회복에 있어 수용전념치료적 접근이 효과적인 방법으로 널리 활용될 수 있으며, 특히 현 연구 결과를 대한민국에서 정신건강 서비스 현장의 적용과 연결해 볼 수 있음을 언급하였다. 마지막으로 그 밖의 연구의 의의와 제한점을 논의하였다.

Abstract

Acceptance and Commitment Therapy (ACT), as a trans-diagnostic approach, has been proven effective in the recovery of psychosis and is being used more and more widely, but there are few related studies in Korea. This study aimed to systematically review the literature from 2002 to 2021 on the application of ACT for participants with psychosis. The data analysis included two folds: (a) the preliminary analysis the literatures (N= 12) to screen the qualitative data for meta-analysis and (b) primary meta-analysis on the relevant literature (n = 8) to determine the effect size in recovery. The results revealed that the effect size of ACT for participants with psychosis was significant. The results suggest that the ACT approach can be a therapeutically effective method for both clinical and personal recovery of a person with psychosis. The significance, limitations, and implications of this study were discussed.
