음악치료 프로그램이 치매노인에게 미치는 효과에 대한 메타분석

A Meta-analysis on the Effect of Music Therapy for Dementia

이 연구는 1998년부터 2014년까지 치매노인을 대상으로 진행된 연구들을 대상으로 음악치료가 치매노인에게 미치는 효과를 체계적으로 분석하고자 메타분석을 실시하였다. 석․박사 논문 20편과 학술지 논문 10편을 대상으로 중재방법, 중재목적, 그리고 총 회기 수, 회기 당 운영시간 등으로 나누어 변인별로 분석하였고 그 결과는 다음과 같았다. 총 30편의 논문에서 107개의 효과크기 사례수가 도출되었으며 전체효과크기는 0.851의 큰 효과크기를 나타냈다. 변인별로 대상의 연령과 증상에 따라 구분하였는데 연령에 따라 80세 이상에서 더 효과적이었으며 증상에 따라 경증에서 큰 효과크기를 보였으나 각각의 집단 간 통계적으로 유의미한 차이를 보이지 않았다. 중재목적에 대해서는 인지, 사회/정서, 행동 세 가지 영역으로 나누어 분석했으며 그 결과 사회/정서영역이 가장 큰 효과크기를 나타냈고 인지영역, 행동영역순의 효과크기를 보였다. 중재방법을 적극적 활동과 혼합적 활동으로 구분하여 효과크기를 비교한 결과 적극적 활동에서 큰 효과크기를 나타냈으나 통계적인 차이를 보이지 않았다. 음악선곡 방법에 따른 분석 결과 치료사가 지정한 경우 큰 효과크기를 보였으며 음악치료사가 시행했을 때 더 효과적임을 알 수 있었다. 끝으로 총 회기 수와 회기 당 운영시간을 분석 한 결과 10회기 이하에서 큰 효과크기를 나타냈으며 회기 당 운영시간 역시 40분미만일 때 큰 효과크기를 나타냈다. 총 회기 수와 회기 당 운영시간에 대해 메타회귀분석 결과 10회기 이하, 40분 미만에서 경향성이 증가하는 모습을 나타냈다.

Abstract

The purpose of this research was to systematically analyze the effects of music therapy for dementia using the studies conducted from 1998 to 2014 on dementia for meta-analysis. 20 doctoral and master theses and 10 published articles were analyzed by dividing into dependent variables, independent variables, the total number of sessions and operating hours per session. 107 effect sizes were derived from 30 studies. The meta-analysis showed 0.851 of overall effect size, which means that music therapy is effective to dementia according to Cohen’s criteria of above 0.8 (1988). The analysis by variables showed that for ages over 80’s effect size was 0.899 while below 80’s effect size was 0.812; regardless of age it showed high effect sizes. Mild symptoms showed higher effective size than severe symptoms but there was no statistical difference between the two degrees of symptoms. Regarding the intervention purpose, dependent variables were analyzed by dividing into cognitive, emotional/social and behavioral areas; emotional/social area’s effect size was highest, followed by the cognitive and the behavioral areas. Regarding the intervention method, active music therapy showed a significant effect size but there was no statistical difference between receptive and active music therapy. If the music was suggested by music therapist, the effect size was higher and more effective. The results of analyzing the total number of sessions and duration per session showed that music therapy is more effective under 10 sessions as well as for a session duration of less than 40 minutes. The meta-regression analysis of the total number of sessions and duration per session showed more effectiveness below 10 sessions and for sessions of less than 40 minutes. This study proves that music therapy is effective for social/emotional, cognitive and behavioral problems in dementia.
