배우자를 돌보는 남성노인에 관한 근거이론 연구: 돌봄의 구조와 유형을 중심으로

초록

본 연구에서는 배우자를 돌보는 남성노인의 돌봄의 구조와 유형을 살펴보고자, 근거이론연구방법을 활용하여 1년 이상 배우자를 돌보고 있는 남성노인 14인과의 심층면접 자료를 분석하였다. 개방코딩을 통해 170개의 개념, 29개의 하위범주, 15개의 범주가 도출되었다. 축코딩 결과 ‘예상치 못한 부인의 발병’, ‘시설에 대한 거부감’, ‘부인 돌봄을 감당하게 됨’ 등이 인과적 조건으로, ‘남편과 가장으로서 책임 수행’과 ‘돌봄자 역할과 관리자 역할’이 현상으로, ‘돌봄의 동기와 경제적 여건’, ‘돌봄 부담과 남성의 어려움’이 맥락조건으로, ‘돌봄 적응 및 숙련’과 ‘가족 유대 및 가족 돌봄 자원’이 중재적 조건으로, ‘보람과 긍정적 의미 찾기’, ‘혼자 참고 견디기’, ‘방치와 체념’이 전략으로 나타났으며, 그 결과는 ‘생활과 돌봄의 안정’, ‘그럭저럭 버티어 나감’, ‘불안정(위험의 내재)’ 등으로 나타났다. 선택코딩 결과 핵심범주는 ‘남편과 가장으로서 책임 다하기’로 파악되었으며, 핵심범주를 중심으로 남성노인의 배우자 돌봄에 대한 이야기 개요를 제시하였다. 배우자 돌봄 유형은 ‘관리자 가장형’, ‘돌봄 수행형’, ‘체념방치형’으로 분류하였다. 남성노인의 배우자 돌봄은 가족관계와 깊은 관련을 가지고 있으며 공적 돌봄 서비스 지원이 남성노인의 돌봄자 역할을 촉진하는 것으로 파악되었다. 가족내 돌봄에서 성뿐 아니라 연령이 주요요인으로 작용하였고, 연구참여자들은 돌봄 부담과 남성으로서의 어려움을 중복적으로 경험하는 것으로 분석되었다. 분석결과를 토대로 가족 단위 개입, 남성노인 돌봄자의 유형별 욕구에 기반한 지원, 생애주기에 걸친 돌봄 교육과 훈련, 예방적 개입이 필요함을 제언하였다.

Abstract

This study aims to analyze the structure, process and types of care of elderly male caregiving by conducting the ground theory method. The data were drawn from in depth interviews with 14 elderly male caregivers who had been engaging in care for their wives at home for more than a year. As the result of the open coding, 170 concepts, 29 sub categories, and 15 categories were emerged. ‘Unexpected illness’ and ‘refusal to nursing home placement' were grasped as causal conditions, ‘taking responsibilities as a husband and a leader of the family' and ‘caregiver role and manager role' as the phenomena, ‘motivations of care and economic conditions' and ‘care burden and difficulties experienced as a men' as the contextual conditions, ‘level of adaptation and skills of care' and ‘family ties and care resources' as the interventional conditions, ‘finding rewards and positive meanings', ‘enduring alone', and ‘neglect and resignation' as the strategies, and ‘stabilized life and care', ‘managing to maintain status quo', ‘unstabilized life and care(latent risks)' as the results. The core category was presented as ‘taking responsibilities as a husband and head of the family'. The caring process was showed as a story line composed around the core category. The types of caring were classified as ‘managerial leader', ‘care-focused practitioner', and ‘resignating neglector'. By and large, elderly male caregiving was demonstrated as a ‘familial process' involving diverse family dynamics and different levels of care resources available. The generation was one of main factors affecting family caregiving situations together with gender. The participants experienced hardships not only as caregiviers also as men during the caring process. Several suggestions were made based on the results of the study concerning appropriate supports for elderly male caregivers and their families considering the uniqueness of their experiences and needs.
