Abstract
The study was designed to examine the effects of combined training and aerobics training on functional fitness, gait, and stability in hemiplegic stroke patients. Forty-five subjects volunteered to participate and were randomized into one of three groups. Thirty-nine subjects completed the study, and the number of subjects in each group was as follows: combined exercise training group (CET: n=13), aerobic exercise training group (AET: n=13), and control group (CON: n=13). Subjects in CET performed treadmill walking(20 min) and resistance exercise(20 min) using Thera-band simultaneously, and subjects in AET performed only aerobic exercise utilizing the treadmill(20 min) and stationary cycle(20 min). Subjects in the training groups performed main exercise for 40 minutes, three times per week for 12 weeks, whereas subjects in CON were asked to maintain their normal life patterns during the intervention period. Variables related to functional fitness, gait, and stability were measured before and after the intervention, and values were compared among groups and between pre- and post-intervention using a two-way ANOVA with repeated measures. Results of the present study were as follows: 1. Regarding the functional fitness variables, there were significant group×test interactions for knee extension strength in non-paretic side, standing up and sitting down a chair, functional reach, and sit-and-reach on chair. The CET showed significant improvements in knee flexion strength in non-paretic side, knee flexion strength in paretic side, knee extension strength in paretic side, standing up and sitting down a chair, functional reach, and sit-and-reach while sitting in a chair, whereas the AET showed significant improvement in only 2.44m up and go. 2. For the gait variables, there were significant group×test interactions in 6m walk, 10m walk, and climbing up 4 steps. The CET showed significant enhancement in 6m walk, 10m walk, and 3m backward walk, whereas the AET showed significant improvement in only 10m walk. 3. No significant change was found in any stability-related variables following all interventions. It was concluded that both the combined and aerobic training program had positive effects on functional fitness for daily living and gait ability in hemiplegic patients. Moreover, combined aerobic and resistance exercise training was more effective in enhancing functional fitness and gait ability than aerobic training. Therefore, resistance exercise as well as aerobic exercise should be included in the treatment for hemiplegic patients later on.
