뇌성마비 아동의 기초개념 이해도 연구

본 연구의 목적은 언어발달이 지연된 뇌성마비 아동을 대상으로 기초개념 이해도 양상을 파악하는 것이다. 뇌성마비 아동의 기초개념 이해도가 어떠한지, 뇌성마비로 인한 운동기능의 제약이 의사소통 발달을 방해하여 초기 개념 형성에 어려움을 초래하는지, 생애 초기에는 개념 형성이 지연되더라도 삶의 유지 차원에서 생활연령이 증가하면 기초개념도 점차 습득되는지 등을 알아보기 위하여, 전문의에 의해 뇌성마비로 진단 받은 전북 지역 거주 6세 미만의 뇌성마비 아동을 대상으로 『K-ABC』, 『REVT』, 기초개념 이해도 검사를 실시하고, 이들 중에서 수용어휘 등가연령이 6세 미만에 해당하는 아동 24명에 대한 기초개념 이해도를 분석하였다. 이에 대한 대조군으로, 전주 소재 유치원 두 곳에서 청력, 인지, 정서적 어려움이 보고되지 않은 3세 이상 6세 미만의 아동 44명을 대상으로 동일한 검사를 실시한 후 검사 결과를 비교 분석하였다. 검사 결과에 대한 분석은 SPSS 18.0을 이용하였다. 연구결과, 뇌성마비 아동은 일반아동에 비해 기초개념 이해도와 습득한 개념 수가 유의미하게 낮았다. 일반아동이 생존 관련 개념 외에 [게임], [악기], [놀이] 등과 같은 다양한 개념을 발달시키는 것과 달리 뇌성마비 아동이 높은 이해도를 보인 개념은 [식생활], [양육자], [위생] 등과 같은 생존 관련 개념에 집중되어 있었다. 또한 지능과 생활연령이 기초개념 이해도에 미친 영향을 보기 위하여 회귀분석을 실시하였을 때, 뇌성마비 아동은 지능과 생활연령이 모두 기초개념 이해도에 유의미한 영향을 미치는 것으로 나타났다. 이와 같은 기초개념 이해도 양상에 대한 연구 결과는 향후 대상 아동의 수를 확대하고 대상 장애 유형을 다양화 하여 연구함으로써 개념 기반 언어치료의 방향을 제시하는 기초자료를 제공할 수 있을 것으로 기대된다.
Abstract
This research analyzed the basic concept understanding aspect of children with cerebral palsy. For that, the study compared two groups: one was composed of 24 children with cerebral palsy, and the other of 44 children without disabilities who their intelligence quotient was equal or above 85 and were not reported any difficulties of hearing, cognition, and emotion. Two groups’ basic concept understanding had been tested and the test results were statistically analyzed by SPSS(version 18). Throughout this study, the average of basic concept understanding score was 2.56 while the average of normally developing children was 3.69. Considering 3 point as the concept acquisition score depending on the criterion of Nelson and Bouvillian(1973), the degree of basic concept acquisition of the children with cerebral palsy was only 49.1% of normal children group. The difference of these two groups was statistically significant. They had some difficulties for the basic concept formation, especially for abstract concept such as [distance], [direction], [degree], [wealth and poverty], [time], [ordinal number], [cereals], [occupation], [physical property], and [contingency]. The intelligence quotient influenced on the conceptual understanding scores in both of two groups, however the age influenced the conceptual understanding scores only in the group with cerebral palsy.
