건강보험보장성 강화정책과 민간보험지출액의 관계분석: 한국의료패널자료를 활용한 DID 분석의 적용

최근 강화되고 있는 국민건강보장성 시책은 시의적절한 것으로 생각된다. 특히, 5대 중증질환의 경우 한번 발병할경우 가족전체의 생계가 위협받는 경우가 대부분이다. 따라서, 앞으로는 경제적으로 가계부담이 큰 질환에 대한 폭넓은 지원이 고려될 필요가 있다. 그렇지만, 건강보험 보장성 정책이 지속적으로 강화되기 위해서는 이 정책들이 적절한 효과를 가져왔는지 검증해보아야 한다. 효과가 검증되지 못한 정책에 지속적인 지원이 이루어지기는 어렵다. 이러한 중요성에도 불구하고 국민의 건강권 확보 및 유지를 위한 의료보험과 관련한 기존 연구들은 건강보험과 민간보험각각에서의 연구들이 지속되어 왔을 뿐, 건강보험과 민간보험 양자 간의 관계를 중심으로 통계적으로 검증하고 효과를 논의한 연구들이 부족하다. 즉, 선행연구들은 건강보험보장성 강화의 효과를 만족도와 건강수준에만 초점을 두는경향이 많았으며, 소수의 민간보험지출 연구에 있어서도 비교대상이 없이 연구한 경우가 많아 정밀한 효과라 판단하기 어려운 경우가 많았다. 이에 본 연구에서는 이중차이모형(DID: differences in differences) 방식을 통해 비교집단을설정하고 정책대상의 효과에 대해 측정함으로서 건강보험보장성 강화정책의 민간보험지출액에 대한 정책효과를 정밀하게 측정해보고자 한다.
Abstract
It is believed that the current policy trend for strengthening the national healthcare insurancecoverage is thought to be an appropriate move. In particular, any one of the five key diseases canthreaten the very economic foundation of a family. Therefore, it is necessary to consider providingeven a broader range of support for the diseases that can become serious economic burdens to families. However, in order to ensure continuous strengthening of the policies on insurance coverage, itis necessary to verify the efficacy of these policies. It would be very difficult to provide continuoussupports to policies whose effects are yet to be verified. In spite of such importance, existing studiesrelated to the healthcare insurance system, which is necessary to secure the right of acquiring andmaintaining a healthy life for everyone, have been conducted by the public healthcare sector and theprivate health insurance sector separately, resulting in the absence of a study that focuses on the relationshipbetween the two with empirical discussions on their effects. Existing studies focus mainlyon the level of health and satisfaction to examine the effects of strengthening healthcare insurancecoverage. As for the studies on the expenditure by private insurers, many of these were conductedwithout decent control groups, rendering the results not sophisticated enough to highlight the trueefficacy of such policies. In this study, we used a DID (differences in differences) model to identify acontrol group and measure the efficacy of the policies on the target audiences, with a view to quantifyingthe effect of the policy to strengthen healthcare insurance coverage on the expenditures madeby private insurers.
