Background : The purpose of this study was to investigate the effects of combined smoking and obesity on Hyper-LDL-cholesterolemia by blood pressure category. Methods : This secondary analysis study used data from the Korea National Health and Nutrition Examination Survey (KNHANES), 2019-2021. A total of 14,322 subjects aged 30 years and older were included. The data were analyzed using descriptive statistics, complex sample cross-tabulation, and complex sample multiple logistic regression analysis. Results : The prevalence of hyper-LDL-cholesterolemia was 62.5% in the hypertensive group and 31.1% in the non-hypertensive group. The risk of hyper-LDL cholesterolemia was elevated in overweight (OR = 1.71, 95% CI = 1.38-2.12) and obese non-current smokers (OR = 1.90, 95% CI = 1.57-2.29) compared with normal/underweight non-smokers. Among hypertensive individuals, the highest odds were observed in obese current smokers (OR = 1.97, 95% CI = 1.42-2.72), while in non-hypertensive individuals, the odds were markedly higher in overweight current smokers (OR = 2.53, 95% CI = 1.86-3.43) and obese current smokers (OR = 3.25, 95% CI = 2.48-4.27). Conclusion : These findings emphasize the importance of blood pressure–specific strategies that integrate smoking cessation and weight management for effective cardiovascular disease prevention.
