알코올과 종교중독 성향 사례 분석: 기독교 상담과 투사적 동일시를 중심으로

본 연구는 교회 활동에 열심을 보이면서도 밤마다 술을 마시고, 이에 따른 죄책감과 자기혐오감에 시달렸던 여성을 정신분석적 대상관계이론의 투사적 동일시 개념에 근거하여 기독교적 입장에서 상담한 과정과 내용을 담은 사례 연구이다. 먼저 알코올 중독과 종교중독 현상을 대상관계적 입장에서 설명하고, 두 가지 장애에 대한 진단기준을 제시하였다. 이어서 10회 상담의 각 회기별 요약과 치료 과정에 따른 변화에 대한 상담자의 분석을 소개하였다. 전체적으로 상담 내용은 자기 표상과 타인 표상, 투사적 동일시 개념을 활용하여 분석되었으며, 치료적 변화가 일어나는 자리에서 상담자가 담당한 대상 경험을 소개하였다. 상담자는 담아내기를 충분히 활용하여 내담자의 수치와 분노, 불안을 잘 견뎌내었으며, 이를 통해 내담자는 어린 시절 외로운 시간, 자신을 있는 모습 그대로 받아주신다고 느꼈던 하나님과의 연결을 되살려내었다. 사례분석은 최영민의 5단계 투사적 동일시 과정을 적용하여 이루어졌다(투사, 투사와의 연결, 조종, 투사한 내용대로 반응, 재내면화). 내담자는 상담자를 향해 분노를 쏟아내기도 하고, 상담자의 옷에 눈물과 콧물을 묻히며 부끄러운 모습을 그대로 드러내면서 이 모습을 평가하지 않고 그대로 받아준 상담자를 통하여 죄인인 자신을 있는 그대로 받아주신 하나님에게 받아들여지는 경험을 하였다. 그 결과 역기능적인 투사적 동일시 기제에 새로운 자기상이 내사되기 시작함으로써 밤마다 마시던 술을 끊고, 중독적 성향의 교회 생활을 조절할 수 있게 되었다. 마지막으로 본 사례 연구의 치료적 성과와 한계를 기술하였다.
Abstract
This study presents a case analysis of a woman in church who has a tendency of alcohol addiction and religious addiction. She had suffered from guilty feeling and self-disgust most of the time because she had been drinking alcohol every night even though she was attending church. Beginning with a brief conceptual explanation of addiction and projective identification, this article suggests diagnostic criteria for alcohol-related disorder and religious addiction. Also, summaries of ten counseling sessions are given followed by the analysis of therapeutic changes. The counselor used 'containing' technique, so she could experience the shame, anger and anxiety of the client. Through the counseling process, the client recovered the feeling of connection with God who accepted her as she was. The case analysis was performed by applying five-stage projective identification model by Choi Y.: projection, keeping connection, controlling, projective counter-identification, re-internalization. Finally, the advantages of working on projective identification in the field of addiction and Christian counseling is discussed along with the strengths and limits of this case study.
