완전 무치악 환자에서 상악 총의치와 하악 임플란트 지지 고정성 보철 치료를 통한 전악 구강회복 증례

치과 분야에서 디지털 시스템의 발전으로 임플란트 치료 시 정확한 진단과 식립, 그리고 그에 따른 원활한 보철 수복이 가능하게 되었다. 무치악 환자에서 임플란트 치료는 불량한 골 위치에 식립 시 합병증으로 인해 예후를 장담할 수 없어 환자의 경제적 사정까지 고려하여 경우 임플란트 치료 대신 총의치 치료를 계획하는 것도 현실적인 방법이다. 본 증례는 완전 무치악 환자에서 상악 총의치와 하악 임플란트 지지 고정성 보철 치료를 시행하였다. 상, 하악의 적절한 이행 의치를 최종 보철물에 준하게 제작하여 상악 최종 보철물 사용 전 사용감을 평가해 볼 수 있었으며 이행 의치를 참고하여 전통적 방식을 따라 상악 총의치를 제작하였다. 하악에서는 이행 의치와 디지털 시스템을 사용해 이상적인 위치로 다수 임플란트의 진단 분석이 가능하였다. 식립 가이드를 제작하여 진단 위치로 고정체 식립을 도왔으며 이행 의치의 정보를 사용하여 적절한 임플란트 보철 수복까지 가능하였다. 상, 하악 최종 보철물 장착 후 만족스러운 치료결과를 보여 이에 보고하고자 한다.
Abstract
With the development of the digital system in dental field, accurate diagnosis, fixture placement, and convenient prosthetic restoration have become applicable during implant treatment. In this case report, maxillary complete denture and mandibular implant supported fixed prostheses were fabricated in an edentulous patient. Appropriate interim dentures of the maxillary and mandibular were fabricated according to conventional method, and to evaluate the usage of complete denture. In mandibular, multiple implants’ diagnostic analysis upon ideal position was possible using interim denture and the digital system. This case is presented as satisfactory treatment results were achieved after the delivery of definitive prostheses.
