피하 알레르겐-특이 면역요법(allergen specific subcutaneous immunotherapy, 이하 면역요법)은 원인 알레르겐을 환자에게 주사하여 면역관용을 유도하는 치료법으로서 초기 치료(build-up phase)를 어떻게 하느냐에 따라서 전통적인 면역요법(conventional immunotherapy), 집중 면역요법(cluster immunotherapy), 급속 면역요법(rush immunotherapy), 초급속 면역요법(ultrarush immunotherapy)으로 나누어진다. 초급속 면역요법은 하루에 유지 용량 바로 전까지 이르도록 주사하는 방법으로서 병원 방문 횟수를 줄여서 환자의 편리성을 높여주고 치료 효과가 하루만에도 빠르게 나타나며 의료진들이 집중하여 주사하기 때문에 약물 용량 관리와 주사 행위에 따르는 부작용을 줄일 수 있는 장점이 있다. 하지만 하루에 치료 용량까지 올려야 하기 때문에 부작용에 대한 우려가 있다. 곤충독 알레르기에서는 초급속 면역요법과 전통적인 면역요법의 부작용 발현이 별반 차이가 없고 벌이나 개미에 재차 물려서 나타나는 아나필락시스로부터 환자를 즉각 보호하기 위해서 전통적인 면역요법보다 급속 혹은 초급속 면역요법이 선호되고 있다. 전신부작용 발현율은 초급속 면역요법이 10.7%–38%로 전통적인 면역요법 0.84%–46.7%와 차이가 없다.1 항히스타민 전처치는 국소부작용을 줄일 수 있지만, 전신부작용과 중증부작용은 줄이지 못하였다.
