외현화 장애 집단의 관리기능 결함 양상

A Study on Externalizing Disorder Patients Group and Its Executive Function Deficiency Conditions

초록

본 연구는 주의력결핍과잉행동장애의 혼합형 및 적대적 반항장애와 품행장애 환자로 구성된 외현화장애 환자군과 정상 대조군을 대상으로 관리기능의 결함에 대해서 알아보고자 하였다. 연구결과를 살펴보면, 연속수행검사와 조건연속수행검사로 측정된 억제능력의 경우 특히 ADHD-C집단이 ODD·CD 및 정상집단에 비해 수행이 저조한 것으로 나타났다. 선 추적검사로 측정된 인지도식의 전환능력의 경우, TMT B의 완료시간 측정치에서 ADHD-C, ODD·CD 각각의 두 집단이 정상집단에 비해 수행이 저조한 것으로 나타났다. 또한 카드분류검사로 측정된 개념형성능력의 경우에는 ADHD-C집단의 수행이 정상군과 비교하여 저조한 것으로 나타났으며, 보속오류수에서 ODD·CD집단과 비교하여 오류수를 더 범한 것으로 나타났다. 다음으로 작업 기억 능력에서는 외현화장애의 각 유형별로 정상군에 비해 수행이 저조하였으나, 각 유형별 수행의 차이는 없었다. 마지막으로 판별함수분석 실시 결과, 억제능력에서 분류정확률이 가장 높은 것으로 나타났다. 본 연구는 국내에서 ADHD의 관리기능에 대해서 많은 연구를 한 것에 더하여, ODD·CD집단을 추가하여 이들의 관리기능을 살피고자 하는 연구로서, ODD·CD의 국내 연구가 상대적으로 미비한 상황에서 이들을 대상으로 하여 관리기능에 대한 연구를 했다는 점과 임상현장에서 보다 구체적인 진단과 예후를 판단하는데 도움이 될 것이라는 점에서 의의가 있다.

Abstract

The study aims to examine the deficiencies in executive functions by comparing the normal control group with externalizing disorder patient group suffering from Attention Deficit Hyperactivity Disorder Combined Type (ADHD-C), Oppositional Defiant Disorder (ODD), and Conduct Disorder (CD). According to the study results, the inhibition ability measured by Continuous Performance Test (CPT) and Conditional Continuous Performance Test (CCPT) was found to function less actively in case of ADHD-C group compared with ODD, CD, and normal groups. Regarding the cognitive schema switching ability measured by Trail Making Test (TMT), the response time measures of TMT B was shown to perform less active in both ADHD-C and ODD and CD groups compared with the normal group. In addition, the concept formation ability measured by Wisconsin Card Sorting Test (WCST) appeared to be less active in case of ADHD-C group compared with normal group, and the number of perseverative error was found to be higher than ODD and CD groups. In case of working memory ability, those suffering from each type of externalizing disorders were shown to underperform relative to normal group, but the performance by each type was significantly indifferent. Lastly, Discriminant Function The results revealed that the classification accuracy in case of inhibition ability was found to be the highest. The study has an implication that the author attempted an experimental research on executive function of ODD and CD groups, which has not been explored yet, by further adding the ODD and CD groups less examined as well as the existing investigation on ADHD group undertaken in Korea and then identifying the executive function found in the additional two groups, which may contribute to the in-depth diagnosis and prognosis judgments in a clinical field.
