폐경기 여성에서 승마물질이 폐경 증상과 골에 미치는 영향

The Effects of Black Cohosh Root Extract on the Vasomotor Symptom and Bone of Postmenopausal Women

초록

목적: 폐경 후 여성에서 에스트로겐 보충요법은 장기적인 면에서 심혈관계 질환 예방과 골다공증의 예방에 긍정적인 효과가 있는 것으로 알려져 있으나, WHI Study 후 환자들의 장기적인 약물투여 순응도가 떨어져 이를 대신할 수 있는 안전하고 새로운 대체요법의 필요성이 절실히 요구된다. 연구자들은 승마추출물이 호르몬 요법과 급성 폐경증상, 골밀도에 대한 효과를 알아보고자 이중맹검, 무작위, 전향적 연구를 시행하였다. 방법: 2002년 6월부터 2004년 6월까지 고신대학교 복음병원 가정의학과 및 산부인과 외래를 방문한 자궁의 양성 질환으로 자궁적출술 및 양측 난소절제술을 시행한 여성 중 승마물질투여를 6개월간 시행한 30명의 폐경 여성(1군)과 에스트로겐 요법을 6개월간 시행한 30명의 폐경 여성(2군), 그리고 폐경이 되었지만 치료를 하지 않은 30명의 여성(3군)을 대상으로 하여, 치료 시작 전, 치료 3개월 후, 치료 6개월 후의 Kupperman index, 골밀도 및 골대사 표지자를 측정하였다. 결과: Kupperman index는 3개월, 6개월째에 1군과 2군에서 통계학적으로 유의한 감소를 보였으며(P<0.05), 치료군 간의 비교에서는 3군에 비해 1군과 2군에서 3개월과 6개월에 각각 통계학적으로 유의한 감소를 보였다(P<0.05). 척추 골밀도는 3개월, 6개월째에 1군, 2군에서 통계학적으로 유의한 증가를 보였으며(P<0.05), 치료군 간의 비교에서는 3개월, 6개월째에 3군에 비해 1, 2군은 유의한 골밀도의 증가를 보였다(P<0.05). 요중 deoxypyridinoline는 3개월, 6개월째에 1군, 2군에서 통계학적으로 유의한 감소를 보였으며(P<0.05), 치료군 간의 비교에서는 6개월째에 3군에 비해 1, 2군에서 유의한 감소를 보였다(P<0.05). 결론: 승마물질이 급성 폐경증상 뿐 아니라 골대사에도 긍정적 효과를 나타내어 치료의 순응도를 높일 수 있는 폐경기 여성에서 새로운 대체요법으로 생각되어진다.

Abstract

Objectives: To evaluate the effect of black cohosh root extract(BCRE) on vasomotor symptoms and bone mineral density in postmenopausal women. Methods: This prospective randomized clinical trial examined the effects of BCRE on vasomotor symptom, bone mineral density, and biochemical markers of bone turnover in 90 postmenopausal women. Treatment included BCRE(group I, n=30) or 0.625 mg CEE(group II, n=30), control group(group III, n=30) for 6 months. Kupperman index, BMD and biochemical bone marker were measured at 0, 3 and 6 months. Results: Kupperman index decreased significantly at 3 months and 6 months of the treatment in group I and group II(P<0.05). BMD of lumbar spine increased significantly during the treatment in group I and group II(P<0.05). Urinary deoxypyridinolin decreased significantly at 3 months and 6 months of the treatment in group I and group II(P<0.05). Conclusion: BCRE appears to be a safe and effective alternate to hormone therapy for vasomotor symptom and may be especially useful in women with intolerance or contraindication to traditional hormone therapy.
