노인의 자살생각 억제와 관련된 변인에 대한 메타분석 - 국내연구를 중심으로 -

본 연구는 지난 10년간의 선행연구를 바탕으로 노인의 자살생각과 관련된 억제변인들의 효과크기를 메타분석을 통해 살펴봄으로 노인의 자살생각과 관련된 후속연구의 방향을 제시하고, 노인의 자살을 예방하는 방안을 탐색 하는데 그 목적이 있다. 본 연구에서는 노인의 자살생각을 종속변인으로 하고, 노인의 자살생각 억제에 영향을 미치는 변인들을 독립변인으로 하는 학술지 및 석·박사논문 45편을 연구대상으로 선정하였다. 연구결과는 다음과 같다. 첫째, 노인의 자살생각과 관련된 변인은 모두 23개로, 개인특성 변인군의 하위변인이 7개, 개인·가족 환경 변인군의 하위변인이 8개, 개인 심리 변인군의 하위변인이 4개, 사회 환경 억제 변인군의 하위변인이 4개로 분류되었다. 둘째, 변인군 별 효과크기는 개인·가족 환경 변인군, 개인 심리 변인군, 사회 환경 변인군 순이었으며, 세변인군 모두 중간 효과크기를 보였다. 개인특성 변인군은 유의미한 효과가 나타나지 않았다. 셋째, 개인·가족 환경 변인군 중 가족 지지가 큰 효과 크기를 나타냈고, 경제적 만족 및 가족관계 만족, 건강 만족, 가족 결속력이 중간 효과크기를 나타냈다. 넷째, 개인 심리 변인군의 하위변인 중 심리적 안녕감과 자아존중감이 큰 효과크기를 갖는 것으로 나타났다. 다섯째, 사회 환경 변인군의 하위변인 중 사회 활동 참여가 가장 큰 효과크기를 나타냈고 그 다음 사회적 지지, 전문가 지지 순이었으며 모두 중간효과 크기를 나타냈다. 이러한 결과는 노인자살 생각 억제를 위한 예방교육 및 정책방안에 있어 개인·가족환경변인인 가족 지지와 개인 심리 변인인 심리적 안녕감과 자아존중감 강화 그리고 사회환경변인인 사회 활동참여에 주안점을 둘 필요가 있음을 시사해준다.
Abstract
The purpose of this study was to conduct a systematic review of previous studies on the suicidal ideation of older adults. The data collected for this study are from 42 articles published in academic journals and 61 masters' theses and doctoral dissertations, all of which were released from 2002 to 2011. Of these studies, 45 articles meet the criteria for inclusion in the meta-analysis. The results of this study are as follows:1. It was classified into 4 variable groups and 23 variables(7 of the personal-characteristic variables, 8 of the personal and family environment variables, 4 of the personal-psychological variables, 4 of the social-environmental variables). 2. Included in the main factors related to the suicidal ideation of older adults were the personal and family environment variable, the personal-psychological variable and the social-environmental variable. The most influential factors among three sets were thepersonal and family environment variable,followed by the personal-psychological variable and the social-environmental variable in order. The effect size of all variables was moderate. The personal characteristics variable did not show a significant effect. 3. The most influential variable related to the suicidal ideation of older adults among the set of the personal and family environment variable was family support followed by family relational satisfaction, health satisfaction, family cohesion, economic satisfaction, friends support. The effect size of family support was large the effect size of family relational satisfaction, health satisfaction, family cohesion, economic satisfaction were moderate; and the effect size of friends support was small. 4. The most influential variable related to the suicidal ideation of older adults among the set of the personal psychological variable was a psychological well-being followed by self-esteem and stress coping. The effect size of psychological well-being and self-esteem was large, and the effect size of stress coping was small. 5. The most influential variable related to the suicidal ideation of older adults among the set of social-environmental variable was social activity participation followed by social support, expert support and neighborhoods support. The effect size of the social activity participation, social support and expert support was moderate and the effect size of the neighborhoods support was small.
