근거이론을 활용한 폭식장애 여대생의 경험에 대한 사례분석

초록

본 연구는 폭식장애를 가진 여대생의 가족치료 상담을 통해 나타나고 있는 클라이언트의 경험을 근거이론을 활용하여 분석한 것이다. 폭식장애를 가진 여대생은 항상 불안하고 집중력이 결여되어 있었으며, 부정적이고 신경이 늘 곤두선 상태에서 생활하고 있었다. 클라이언트의 부모는 예민한 성격의 소유자들로 부부싸움이 잦았고, 클라이언트를 형제들과의 관계에서 차별하였으며, 일관적이지 않은 태도로 양육해 왔다. 클라이언트는 가정 내에서 스트레스를 받으면 무의식적으로 먹는 것에 손이 가며 보통 폭식을 하여 이틀치정도의 음식을 먹었다. 또한 클라이언트는 어릴 적부터 문을 여러번 여닫거나 불을 여러번 켰다 껐다하는 강박관념을 가지고 있었고, 학교생활에서 왕따를 당하기도 하였으며, 환청경험도 가지고 있었다. 폭식장애를 가진 여대생의 가족치료 상담 총 12회기의 축어록을 중심으로 근거이론을 활용하여 분석한 결과 361개의 의미 있는 진술을 얻을 수 있었고, 이를 바탕으로 비슷한 개념들을 묶어 11개의 하위범주로, 다시 5개의 상위범주로 나누어 살펴볼 수 있었다. 본 연구를 통해 기존의 폭식장애를 바라보고 있는 개인적 접근에서 벗어나 가족치료적 접근이라는 새로운 방향을 제시하는데 도움이 될 것이다.

Abstract

This research is a case study relating to undergraduate female student with bulimia nervosa. The subjects of the research were consisted of four family members, who were a mother, a client, an elder sister, and a younger brother. The researcher tried to find the factors to influence on bulimia nervosa of female student. The total family counseling was 11 sessions, which included individual and family counseling. The duration of counseling ranged from April, 2002 to August, 2002. The researcher used consistent comparison to find redundant concepts in the case and triangulation method by professional group to increase the reliability of data. The researcher had five meetings to discuss the findings of the research with a therapist who treated the client and her family. The researcher used grounded theory to analyze the qualitative data. Specifically, the researcher utilized open coding, axial coding and selective coding to find the factors, which had influenced on the client's bulimia. The researcher found 361 statements as open codings, and categorized 12 dimensions. The 12 dimensions was categorized 5 categories as axial codings. The 5 axial codings included distrust of parent, divorce problem of parent, difficulties of interpersonal relationships, sexual fear, and sensitive personality. The researcher found an anxiety, which influenced from 5 factors mentioned above, as selective coding. The researcher concluded that the bulimia of the client occurred by the anxiety, which were influenced by 5 factors.
