조기 정신증 개입 서비스 공급 특성에 따른국제 비교 연구

초록

이 연구는 8개국 조기 정신증 개입 서비스를 유형화 하고, 유형별 특징을 비교하기 위해 수행되었다. 연구대상 국가를 선정하기 위해 의료보장 체계를 4가지 유형으로 구분하였고, 각 유형에 속한 국가 중 조기 정신증 개입 서비스를 시행하고 있는 8개국 8개 서비스를 선정하였다. 분석을 위해 서비스 접근성과 서비스의 질을 지표로 선정하고, 지표별로각 4개의 하위요소를 구성하였다. 분석 방법으로는 국가 비교와 유형화 연구에 유용한 것으로 알려진 퍼지셋 이념형 분석을 활용하였다. 연구결과, AQ(보편적 고품질) 모형에는 호주 OYH, 캐나다 PEPP, 영국 LEO가 포함되었고, Aq(보편적 저품질) 모형에는 홍콩EASY, 덴마크 OPUS, 일본Il Bosco가 속하는 것으로 나타났다. aQ(선별적 고품질) 모형에는 싱가포르 EPIP와 미국 OASIS가 포함되었다. 조기 정신증 개입 서비스의 국가간 비교 연구는 각국의 조기 정신증 개입 서비스를 이해하는데기여하고, 국가 차원에서 조기개입 서비스를 시행하려는 국내 정책에 함의를 제공할 것이다.

Abstract

The purpose of this study was to explore and stereotype the introduction, progress and current state of early psychosis intervention service of eight country and thereby explain their characteristics and differences by service type. In order to select the research subject country, the medical security system was divided into 4 types. We selected eight services in eight countries that are providing early psychosis intervention services among nations belonging to each category. For measurement, accessibility to service and quality of service were selected as analysis indexes based on related theories, literature and actual service contents. Each index was made up of four sub-elements which were converted to fuzzy scores of belonging. The study results are follows. First, OYH of Australia, PEPP of Canada, and LEO of the UK turned out to belong to the ‘common high-quality model’ where both accessibility to service and quality of service were high. Second, EASY of Hong Kong, OPUS of Denmark and Il Bosco of Japan turned out to belong to the ‘common low-quality model’ where accessibility to service was high but quality of service was low. Third, EPIP of Singapore and OASIS of the US turned out to belong to the ‘selective high-quality model’ where accessibility to service was low but quality of service was high. The study results would contribute to understanding early psychosis intervention service of each country based on the comparative research and provide implications for domestic mental health policies related to detection and intervention of early psychosis.
