임신성 당뇨병 임부를 위한 통합 자가관리 프로그램 개발과 운영평가

본 연구는 임신성 당뇨병 임부가 임신 중 철저한 교육을 통해 대상자의 임신과 혈당관리를 할 수 있는 능력을 극대화시키는 체계적인 교육과 상담을 시행하고, 내과와 산부인과로 분리되어 관리됨으로 초래되는 취약성을 극복하는 대안으로 통합 자가관리 프로그램을개발하고 개발된 프로그램의 운영평가를 조사하였다. 통합 자가관리 프로그램은 임신 관리와 혈당 관리를두 영역으로 구분되며 임신성 당뇨병에 대한 교육과 식이, 운동, 스트레스 관리, 자가혈당 측정 및 복식호흡의자가관리를 하도록 대상자에게 교육, 정서적 지지를 할뿐 아니라 대상자가 임신성 당뇨병 관리에 대한 의사결정에 참여 하도록 전문가와 상호작용 하는 프로그램을 의미한다. 이는 임신성 당뇨병을 진단받은 임부에게산과전문 간호사가 1시간씩 3회의 소그룹 모임과 10~ 15분간 2회의 전화 상담을 하는 총 5회의 프로그램을통해 제공한다. 통합 자가관리 프로그램의 운영에 대한 평가로 임신성 당뇨병 임부의 내용 만족과 이해도, 진행과정 및 프로그램의 유용성에서 긍정적인 평가하였다. 본 연구는 임신성 당뇨병 임부의 혈당관리와 임신관리를 위한 간호중재 프로그램으로서 처음 개발되고 운영 평가를 시행하였다는 점에서 의의가 있다. 또한 임신성 당뇨병 임부가 자가관리 수행을 극대화하여 혈당관리와 임신 관리를 증가를 위한 통합 자가관리 프로그램이 타병원이나 지역사회에서 활용되어 임신성 당뇨병 임부의 건강관리에 기여할 것이라고 본다. 본 연구 결과를 토대로 다음과 같이 제언을 하고자한다. 첫째, 개발한 통합 자가관리 프로그램을 타병원이나지역사회에서 임신성 당뇨병 관리가 취약한 임부를 대상으로 자가관리, 혈당 및 어머니 역할에 미치는 효과에 대한 연구를 제언한다. 둘째, 무작위 표집 된 임신성 당뇨병 임부를 대상으로 통합 자가관리 프로그램이 혈당, 어머니 역할 및 임신 결과(pregnancy outcomes)에 미치는 효과에 대한 연구를 제언한다. 셋째, 대상자들이 궁금한 것을 상담하고 신뢰 할 수있는 온라인 프로그램의 개발과 효과평가를 하는 연구를 제언한다. 넷째, 대상자들의 임신성 당뇨병 관련 치료 정보검색으로 인터넷으로 쉽게 접근하고 이용할 수 있는 온라인 교육과 상담 프로그램 개발과 효과 평가를 하는연구를 제언한다.
Abstract
Objectives: The purpose of the study was to develop and evaluate an integrated self-management program which could cover expected weakness from separately controlled obstetrics and internal medicine departments in women with gestational diabetes mellitus. Methods: The integrated self-management program was developed to synthesize contents and methods in the management of glycemic control and pregnancy management approach. Results: The program was conducted 3 out of 5 times for 1 hour small group meeting and 2 out of 5 times by telephone-counseling. The integrated self-management program was verified by an expert panel. The integrated self-management program for GDM women showed positive outcomes in satisfaction and understanding of content, program process, and utility. Conclusion: These findings suggest that an integrated self-management program for GDM women has positive effects. Further studies are needed to identify the effects of an integrated selfmanagement program on glycemic control, maternal role, and pregnancy outcomes.
