Abstract
-Purpose:This study investigates the spatial association between the Local Extinction Risk Index (LERI) and per capita medical expenditure across 229 si-gun-gu in South Korea in 2021. Ordinary least squares (OLS) regression, spatial autocorrelation diagnostics, and geographically weighted regression (GWR) were applied sequentially. -Methods:The dependent variable was medical expenses per capita (in 1,000 KRW), and the key independent variable was LERI, which is constructed so that lower values denote higher extinction risk. The current smoking rate, subjective health level, physician density, the financial independence ratio, and the proportion of the social welfare budget were included as control variables. -Results:LERI was negatively associated with per capita expenditure (β=−1114.54, p<0.01), indicating that regions facing population decline and accelerated ageing bear disproportionately higher expenditure burdens. The smoking rate (β=−27.60) and the financial independence ratio (β=−6.58) were also negatively associated, whereas physician density was positively associated (β=14.84). Significant residual spatial autocorrelation (Moran’s I=0.45, p<0.01) and a significant Koenker statistic (28.51, p<0.01) indicated both spatial dependence and spatial non-stationarity, which motivated the use of a local model. GWR revealed pronounced heterogeneity: the association was stronger in depopulated inland areas and weaker in metropolitan regions where supply-side conditions are more influential, and the adjusted R² improved from 0.7191 to 0.9038. -Conclusion:Regional variation in expenditure reflects the spatial interplay of population structure and the distribution of healthcare resources. These findings should be interpreted within the context of a single-payer system operating under a uniform fee schedule. They support place-based policies that combine primary care and long-term care in high-extinction-risk regions with demand-side management in resource-concentrated regions.
