Abstract

-Purpose:This study aimed to compare suicide mortality trends between Jeju Special Self-Governing Province and South Korea nationally over a 29-year period (1996–2024) and to identify vulnerable subgroups through sex- and age-group-stratified analyses. -Methods:Suicide mortality data for individuals aged 15 years and older were extracted from the Cause of Death Statistics via the Korean Statistical Information Service (KOSIS), National Data Agency, stratified by sex and 5-year age group. Age-standardized suicide mortality rates were calculated using the WHO 2000 World Standard Population, and joinpoint regression analysis was used to identify trend changes and estimate annual percent change (APC). Average annual percent change (AAPC) over identical periods was used to directly compare trends between Jeju and the rest of the country (nationwide data excluding Jeju), thereby avoiding the sampling overlap inherent in comparing Jeju with the national total. -Results:Over the 29-year period, 343,153 suicide deaths occurred nationally and 4,507 in Jeju. National male suicide mortality declined significantly after 2009 (APC −1.93%, p<0.05), whereas Jeju males showed no joinpoint and increased significantly throughout the study period (APC +0.69%, p<0.05), reaching 56.2 per 100,000 in 2024—38.7% higher than the national rate. This divergence was most pronounced in the recent period, with Jeju males differing significantly from the rest of the country after 2016 (AAPC difference 2.65 percentage points, p<0.0001). Age-group comparisons showed no significant national-Jeju differences among young and middle-aged males or young females, whereas among older adults (55–74 years), the national decline was significantly steeper than Jeju's (p=0.015). National females showed a non-significant re-increase after 2016, and Jeju females showed no clear significance following joinpoints in 2011 and 2016. -Conclusion:These findings suggest that national suicide prevention policies have not been sufficiently effective in Jeju, particularly in recent years, underscoring the need for region-specific prevention strategies and surveillance systems.
