한국 노인돌봄의 제도적 배열: 돌봄의 공식화를 중심으로

노인돌봄 서비스는 노인장기요양보험제도 도입 이후에도 중앙정부와 지방정부의 사업으로 꾸준히 확장되어 왔다. 노인돌봄 서비스를 제공하는 사업들의 재정과 대상, 제공기관이 별도로 존재하며 이들 간 연계가 쉽지 않다는 점에서 노인돌봄 전달체계는 분절성, 비연속성, 파편성을 갖는다고 진단된다. 선행연구들은노인돌봄 서비스의 이런 문제를 전달체계의 조직적 차원에서 진단하고 진입을일원화하거나 통합 기능을 하는 조직의 신설과 개편을 제안해 왔다. 본 연구에서는 돌봄의 특성과 공급을 둘러싼 이론적 검토를 통해 노인돌봄 서비스를 통합적으로 보장하기 위해서는 내재적인 공급 규칙과 돌봄 관계의 통합성을 확보하는것이 중요하다고 보았다. 8종의 노인돌봄사업의 제도적 배열을 돌봄수급권과 돌봄노동의 공식성을 기준으로 분석했다. 그 결과 노인돌봄 서비스의 서비스 신청과 수급자격 기준이 모두 상이하다는 점 그리고 돌봄 제공 인력의 자격 기준과 보상 기준이 사업별로 달라진다는 점이 돌봄 서비스의 통합적 보장에 걸림돌이 된다고 보았다. 또한 현재 가장 큰 비중을 차지하는 노인장기요양 서비스와 바우처방식의 노인돌봄 서비스에서 이용자와 제공자, 제공 조직 모두가 경제적 이해를추구하면서 형성하는 상업적 돌봄 관계가 돌봄의 연속성(continuum of care)을구축하는 데 부응하지 않는다는 점을 밝혔다. 노인돌봄 서비스의 내재적 통합성일반논문한국 노인돌봄의 제도적 배열 301 을 높이기 위한 재구조화는 사업 간 신청기준과 수급자격을 통일하고 상업적 돌봄 관계 외에 공공서비스 관계를 비롯한 다양한 돌봄 관계가 이용자를 중심으로형성될 수 있도록 정책을 개선할 것을 제안했다.
Abstract
Elder care services have been steadily expanding at central and local government levels even after the introduction of Longᐨterm care Insurance for the elderly. A variety of elderly care programs have been running with provider organizations, care workforce and financial sources all different. In the previous research, the problems of the elder care service have been diagnosed from the perspective of the organizational approach. The approach proposes the establishment of the organization which unifies the gate for care application or integrates separate provision of care services. This article has analysed present elder care programs based on care rights and care relationship. Various care services, including longᐨterm care Insurance services, care vouchers and local care services, were different in terms of application process and eligibility criteria. In addition, in the longᐨterm care service and the voucherᐨtype geriatric care service, which comprises the largest portion of care services, users, care workers and provider organizations form the commercial care relationship whereby they pursue their own interests. The restructuring of the elderly care service needs to be promoted through intrinsic integration so that application processes and eligibility criteria are unified. Various care relationship should be formed around care users to build integrated care provision.
