Abstract
"Purpose: This study aimed to quantify the impact of gingival embrasure size and interdental papilla height on esthetic perception and compare differences between dentists and laypersons, as well as among age groups within laypersons. Materials and Methods: Digital photographs of maxillary anterior teeth were edited to create a baseline image (BL) and six modified images (GE-1/2/3, PH+1/2/3). Forty-three dentists and 119 laypersons (aged 20 - 34, 35 - 49, and 50 - 64 years) evaluated esthetic appeal using a 7-point Likert scale. Gingival gap change (GC) and papilla height change (PC) were compared using paired t-tests. Mean difference of sensitivity (MD), overall mean (OM), and threshold change (TC) were analyzed. Results: Gingival embrasure decreased esthetics 3.9 times more than papilla elongation (GC: -2.37, PC: -0.61, P < .001). Dentists showed significantly lower OM than laypersons (P < .001). Within laypersons, OM increased significantly with age (P < .001). Dentists and younger age groups detected esthetic deterioration at -1 mm gingival embrasure, while older groups recognized it from -2 mm. Conclusion: Gingival embrasure has a far more negative impact on esthetics than papilla elongation. Dentists are more sensitive than laypersons, and younger individuals are more sensitive than older ones to soft tissue changes. Age-specific customized esthetic treatment planning is essential, with particular emphasis on prevention and management of gingival embrasure."
