보톨리눔 톡신을 이용한 교합력 조절과 교합수직고경 증가를 동반한 완전구강회복 증례

병적 치아 마모와 구치부 지지 소실이 동반된 환자의 완전 구강 회복을 위해서는 교합수직고경(Occlusal Vertical Dimension, OVD)의 증가와 안정된 교합의 형성이 필수적이다. 그러나 OVD가 감소한 환자에서 OVD의 회복은 교합력을 상승시켜 수복물의 안정성을 저해할 수 있다. 본 증례에서는 OVD를 4 mm 증가시킨 후, 임플란트 및 치아 지지 보철물을 이용한 완전 구강 회복을 시행하였다. 임시 수복 단계에서 평균 교합력이 보고된 평균값을 상회하는 수준으로 측정되었으며, 이에 따라 교합력을 조절하기 위해 교근에 보툴리눔 톡신 A형(BTX-A)을 주입하였다. OVD 증가와 보툴리눔 톡신을 활용한 교합력 조절을 병행함으로써, 기능적이고 심미적인 완전 구강 회복을 성공적으로 이룰 수 있었다.
Abstract
To rehabilitate a patient with pathological tooth wear and posterior support loss, increasing the occlusal vertical dimension (OVD) and achieving stable occlusion are essential. However, an increase in OVD may elevate occlusal force and compromise restoration stability. This case involved full-mouth rehabilitation using implant- and tooth-supported prostheses following a 4 mm OVD increase. During the provisional phase, occlusal force was controlled via botulinum toxin type A (BTX-A) injection into the masseter muscles. The combined approach of vertical dimension increase and occlusal force management with BTX-A allowed for a stable, esthetic, and functional outcome in full-mouth rehabilitation.
