Objective: To describe the clinical improvement of traditional Korean medicine for treating abducens nerve palsy caused by ruptured vertebral artery dissecting aneurysms. Methods: Patients received conventional treatment followed by acupuncture, herbal medicine, and electroacupuncture. Clinical outcomes were assessed by evaluating ocular motility and the presence of diplopia. Results: During treatment, the extraocular movement improved from 2 mm (OS) to 8 mm (OD) to a nearly normal range. Diplopia severity decreased markedly, and the esotropia and diplopia symptoms subsided noticeably. The Numeric Rating Scale for dizziness decreased from 7 to 1. The patient's functional status improved with the resolution of diplopia during gaze, allowing unrestricted performance of daily activities. No other adverse effects were reported throughout the treatment, except for bruising after acupuncture treatment. Conclusion: Traditional Korean medicine, especially electroacupuncture, may be a useful adjunct for recovery from abducens nerve palsy after vertebral artery aneurysm rupture.
