초록

여러 사람들과 함께 살아가는 사회에서 면역력을 회득하기 전 전염성 질환에 노출되는 경우가 흔히 있다. 이런 경우 즉각적인 적절한 대처를 통하여 질병으로 이환될 가능성을 줄이는 것도 의료인의 의무이다. 인플루엔자 바이러스에 노출되었을 때 zanamivir 혹은 oseltamivir를 투여하고, AIDS (acquired immune immune deficiency syndrome) 환자와 접촉하였을 때는 노출정도에 따라 항바이러스제제 2～3가지를 24시간 이내에 바로 시작하여 4주간 투여하여야 한다. A형 간염은 접촉한 지 2주 이내 면역 글로부린(immunoglobulin)을 0.02 mL/kg 투여하고, B형 간염은 면역 상태에 따라 HBIG (hepatitis B immunoglobulin) 및 B형 간염 백신을 투여하지만, C형 간염은 접촉 후 예방 방법이 아직까지 개발되지 않았다. 성접촉을 통한 급성 매독에 노출되었을 때는 benzathine penicillin G 240만 단위를 일회 근육주사하고, 만성 매독에 노출되었을 때는 혈청학적 검사의 결과에 따라 계획을 세우며, 의료종사자에서 주사 좌상으로 전파되는 경우는 드물다. 가족 내 결핵환자가 생겼을 때 예방적 화학요법의 대상은 6세 미만이며, BCG 접종유무 및 결핵환자의 객담 도말 상태에 따라 달리 Isoniazid를 투여한다. 면역력이 없는 상태에서 홍역이나 수두에 노출되었을 때는 접촉 3일 이내 백신 접종을 하여 경미한 상태로 줄여주며, 면역 기능이 좋지 않아 질병으로 인한 합병증의 위험성이 높을 때는 홍역은 6일 이내 면역글로부린을, 수두는 4일 이내 VZIG (varicella zoster human immune globulin)를 투여한다. 파상풍은 상처의 상태나 파상풍 백신 접종 상태에 따라 Td 혹은 TIG (tetanus human immune globulin)를 주사하고 우리나라에 Td가 시판되고 있으므로 매 10년마다 추가 접종을 하도록 권유한다.
