Abstract
Pancreatic ductal adenocarcinoma (PDAC) is known for its aggressiveness and poor prognosis and often metastasizes to common sites, including the liver, peritoneum, and distant lymph nodes. PDAC may metastasize to the female genital tract, particularly to the uterus; however, this is extremely rare. We report a case of a 63-year-old woman with recurrent PDAC presented as an isolated uterine metastasis, initially presented with abnormal vaginal bleeding. Despite preoperative chemotherapy followed by radiotherapy and surgical resection for PDAC, the patient developed recurrence manifested in the uterineendometrium and cervix, and diagnosis was confirmed based on histopathological evaluation, whichshowed characteristics of PDAC. We performed hysterectomy followed by chemotherapy, which resultedin a complete response. This case report highlights the importance of considering uterine metastasis inthe differential diagnosis in patients with unusual vaginal bleeding and a history of extra pelvic malignancies. Additionally, we have outlined the challenges in distinguishing between primary and metastatic uterine tumors and the need for tailored therapeutic strategies.
