징병신체검사에서 의뢰된 성인 약시 환자의 무늬시유발전위의 양상

목적: 징병신체검사에서 의뢰된 약시 환자의 무늬시유발전위(pattern VEP)를 분석하여 봄으로써 성인 약시 환자의 무늬시유발전위의 양상을 알아보고자 한다. 대상과 방법: 2004년 1월부터 2009년 5월까지 병사용 진단서 발급을 위해 본원 안과 외래에 방문한 20세 이상 남자 67명을 대상으로 의무기록을 토대로 후향적 관찰하여 약시 종류에 따른 시력 및 무늬시유발전위의 진폭 및 잠복기를 분석하였다. 결과: 67명 중 비약시 환자는 13명, 약시 환자는 54명이었으며 약시 환자 54명 중 양안성 약시는 23명, 단안성 약시는 31명이었다. 양안성 약시는 원시성이 4명, 근시성이 7명, 난시성이 12명이었다. 단안성 약시 중 부등시성은 15명, 사시성은 12명, 기질성은 4명이었 으며 양안성 약시 환자의 평균 최대 교정 시력은 0.49였으며 단안성 약시 환자의 약시안의 평균 최대 교정 시력은 0.25였다. 16×16에 서는 진폭 및 잠복기 모두 정상안과 약시안의 차이가 통계적으로 유의하지 않았지만(p＞0.05), 32×32에서는 진폭 및 잠복기 모두에서 정상안과 약시안의 통계적으로 유의한 차이를 보였다(p＜0.05). 약시안과 비약시안의 진폭 및 잠복기의 차이는 격자크기에 따라 유의 하게 차이를 보였으며 약시 종류에 따른 차이는 진폭, 잠복기 모두 차이를 보이지 않았다. 시력에 따른 진폭은 유의하게 양의 관계를 가졌다. 결론: 징병신체검사에서 의뢰된 성인 약시 환자에서 무늬시유발전위검사는 32×32의 격자크기를 이용하여 검사할 경우 약시 진단에 도움을 줄 수 있으며 잠복기와 진폭의 분석을 통해 시력과 약시와의 관계를 알아볼 수 있을 것이라 생각된다.
Abstract
Purpose: To evaluate the pattern VEP in adult amblyopic patients seen in consultation for ophthalmic evaluation as a past of physical examinations for conscription. Methods: We retrospectively analyzed, 67 men, 20-year-old or older, who had pattern VEP done for the diagnosis of amblyopia from January 2004 to May 2009. P100 latency and P100 amplitude were analyzed. Results: Thirteen patients were non-amblyopic, and 54 patients had amblyopia. Binocular amblyopia and monocular amblyopia were found in 23 and 31 patients, respectively. In the binocular amblyopic patients, four patients were hyperopic, seven patients were myopic, and 12 patients were astigmatic amblyopia. In the monocular amblyopic patients, 15 patients were anisometropic, 12 patients were strabismic, and four patients had organic amblyopia. The value of P100 latency and P100 amplitude were statistically significantly different between non-amblyopic and amblyopic eyes, with check size of 32×32. However, the types of amblyopia among the patients were not different at a statistically significant level. Visual acuity and P100 amplitude were significantly positively correlated. Conclusions: VEP might be a useful tool for diagnosis of adult amblyopia, especially using a 32×32 check size. This tool may impart the ability to decide relationship between amblyopia and visual acuity by analyzing P100 latency and amplitude values.
