목적: 맥락막으로의 전이가 드문 신세포암의 맥락막 전이 1예를 보고하고자 한다. 증례요약: 3년 전 우측 신세포암으로 근치적 우신적출술을 시행 받은 과거력이 있는 61세 남자환자가 한 달 전 발생한 우안 시력저하를주소로 내원하였다. 최대교정시력 우안 0.2, 좌안 1.0이었다. 안저검사에서 우안 이측 부위에 황반부를 포함하는 둥근 모양의 노란색융기된 병변이 5.0×4.0 유두직경 크기로 관찰되었으며, 융기된 병변 주위로 장액성 망막박리가 동반되었다. 전신검사에서 뇌와 폐의다발성 전이병변이 발견되었다. 이에 신세포암의 맥락막 전이로 진단하고 sorafenib 경구 투여와 함께 맥락막 전이에 대한 국소 치료로 테논낭하 트리암시놀론 주입술, 유리체강내 라니비주맙 주입술을 시행하였다. 종양의 성장이 진행하고 시력은 안전수동으로 저하되어 안구적출술을 권유하였으나 환자가 원하지 않아 경동공온열요법 시행 후 외래 경과관찰 중에 있다. 결론: 안과적 증상이 원발암 재발의 첫 증상으로 나타날 수 있으므로 전신검사를 통한 전이 종양에 대한 평가가 필요하다. 신세포암의맥락막 전이에 대한 예가 아직 국내에서 보고된 바 없기에 문헌고찰과 더불어 이를 보고하는 바이다.
Abstract
Purpose: To report a rare case of choroidal metastasis from renal cell carcinoma. Case summary: A 61-year-old man visited our clinic with the chief complaint of decreased vision in the right eye. Three years earlier, he underwent right nephrectomy due to renal cell carcinoma. Best corrected visual acuity was 0.2 in the right eye and 1.0 in the left eye. Fundoscopic examination of the right eye revealed a yellowish, dome-shaped elevated choroidal mass with serous retinal detachment, measuring 5.0 × 4.0 disc diameter and located lateral to the macular area. Systemic evaluations showed multiple lung and brain metastases. A diagnosis of choroidal metastasis from renal cell carcinoma was made for the right eye, and the patient received local treatments with oral sorafenib therapy, composed of subtenon triamcinolone injection and intravitreal ranibizumab injection. Tumor progression continued, and visual acuity declined to hand motion. Enucleation was recommended, but the patient refused and is on a regular follow-up after transpupillary thermotherapy. Conclusions: Ocular manifestation may be the initial presenting sign of a recurrent tumor, and an extensive systemic evaluation for metastatic malignancy should be performed. The present example showed a rare case of choroidal metastasis from renal cell carcinoma.
