단안 실명 환자에서 실시한 건안 백내장 수술의 임상성적

목적: 단안 실명 환자에서 실시한 건안 백내장 수술 결과 및 합병증을 정상인의 백내장 수술과 비교하여 합병증과 임상적 유용성을알아보고자 하였다. 대상과 방법: 노년 백내장으로 수정체 유화술 및 인공 수정체 삽입술을 시행 받고 6개월 이상 경과 관찰이 가능했던 84명(84안) 중단안 실명 상태로 건안에 백내장 이외에 시력 저하의 원인이 없는 42명(42안)을 A군, 반대안이 정상인 42명(42안)을 B군으로 구분하여각군에서 술 전, 술 후 시력, 수술 시간, 핵 혼탁 정도 및 합병증 유무 및 빈도를 비교하였다. 결과: 술 전 핵 혼탁 정도, logMAR 시력, 술 후 난시값은 유의한 차이가 없었고 술 후 6개월 후 시력은 두 군 다 술 후 유의한 시력향상이 있었으며(p˂0.01, 0.01) 두 군 사이 술 후 시력의 유의한 차이는 없었다. 합병증은 A군에서 2례, B군에서는 총 4례가 발생하였으며 두 군 사이 합병증 발생에는 유의한 차이가 없었다(p˂0.05). 결론: 단안 실명 환자의 건안에서 백내장 수술을 시행 하였을 경우 유의한 시력상승을 얻을 수 있었으며 정상 백내장 환자의 수술결과와 차이가 없었다.
Abstract
Purpose: To investigate complication rates and results of cataract surgery in a monocular patient compared to normal binocular cataract patients. Methods: We retrospectively analyzed 84 eyes of 84 patients that had undergone phacoemulsification, intraocular lens implantation and were followed up for 6 months or more. The fellow eye condition was classified into two different groups:Group A (experimental group) and Group B (control group). The best corrected visual acuity (BCVA) before and after surgery,operation time, nucleus sclerosis, and complication rate between the two groups were compared. Results: Both groups showed a significant level of visual improvement after 6 months (p < 0.01, 0.01). There was no significant difference in sex, age, preoperative lens opacity, pre- and postoperative visual acuity, spherical equivalent, astigmatism,and rate of complications (posterior capsule rupture [n = 1], uveitis [n = 1] vs. posterior capsule rupture [n = 3], intraocular lens capture [n = 1]. Conclusions: The present study showed no difference in surgical results between Groups A and B. The results demonstrated that cataract surgery in a monocular patient resulted in favorable visual improvement.
