성인에서 눈꺼풀속말림과 눈꺼풀겉말림 환자에서 발생원인과 치료방법에 대한 분석

목적: 본원에서 눈꺼풀속말림 또는 눈꺼풀겉말림으로 수술받은 50세 이상의 성인들을 대상으로 환자수, 발생원인 및 치료방법에 대하여 알아보고자 하였다. 대상과 방법: 2009년 1월 1일부터 2012년 12월 31일까지 본원에서 눈꺼풀속말림으로 수술받은 397명(490안), 눈꺼풀겉말림으로 수술받은 109명(138안)에 대해 발생원인, 치료방법을 후향적으로 분석하였다. 결과: 같은 기간 동안 수술받은 환자 중 눈꺼풀속말림으로 교정수술을 받은 환자수가 눈꺼풀겉말림에 의한 경우보다 3.5배 가량 많았으며 원인으로는 눈꺼풀속말림은 퇴행성(98.4%), 반흔성(1.4%), 연축성(0.2%), 눈꺼풀겉말림은 반흔성(60.1%), 마비성(23.2%), 퇴행성(16.7%) 순서로 나타났다. 결론: 눈꺼풀속말림은 퇴행성변화, 눈꺼풀겉말림은 반흔성 변화가 주된 발생원인으로 나타났다. 또한 퇴행성 눈꺼풀속말림의 빈도가퇴행성 눈꺼풀겉말림의 비해 현저히 높은 것은 동양인의 눈꺼풀 해부학적 구조와 관련 있을 것으로 생각한다.
Abstract
Purpose: To evaluate the causes and treatments of entropion and ectropion in adults. Methods: A retrospective review was performed of 397 patients (490 eyes) with entropion and 109 patients (138 eyes) with ectropionwho underwent corrective surgery at Kim's Eye Hospital from January 2009 to December 2012. Results: The surgical correction of entropion was 3.5 times greater than that of ectropion. The causes of entropion were classifiedas senile (98.4%), cicatricial (1.4%) and spastic (0.2%). The causes of ectropion were classified as cicatricial (60.1%), paralytic(23.2%) and senile (16.7%). Conclusions: The main causes of entropion and ectropion were senile and cicatricial, respectively. Specifically, senile entropionwas more common than senile ectropion, which is considered to be related to the lid anatomy of Asian patients.
