병변내 시도포비어 주입술에 효과를 보인 재발성 눈물낭 기원편평상피유두종 1예

목적: 수차례의 완전절제술 후에도 잦은 재발을 보이는 눈물낭 기원 편평상피유두종 환자에게 병변내 시도포비어 주입술을 병용하여효과를 보았던 1예를 보고하고자 한다. 증례요약: 본원에 내원하기 9개월 전, 타 병원에서 종괴 절제술을 시행하였으나 2개월 전부터 같은 위치의 좌측 안쪽 눈구석의 재발된종괴로 59세 남자 환자가 내원하였다. 안와 전산화단층촬영상 눈물낭에서 시작하여 내측 안와벽 주변에 위치한 종괴가 관찰되었다. 절제 생검을 통한 병리조직검사상 편평상피유두종으로 진단되었다. 이후 1차례의 완전절제술을 시행하였으나 6개월의 간격을 두고반복되는 재발을 보였고, 단순 수술적 제거로는 완치가 어려울 것으로 생각하여 병변내 시도포비어 주입술을 병용하기로 하였다. 수술 시부터 3주 간격으로 5 mg/mL의 농도로 병변내 시도포비어를 주입하였다. 술 후 2달째, 3차례의 병변내 시도포비어 주입술 시행후 일시적으로 윗눈꺼풀에 재발소견을 보였으나 지속적인 병변내 시도포비어 주입술 후 호전되었다. 총 9회의 병변내 시도포비어주입술 후 12개월의 추적 경과관찰 기간 동안 새로운 병변이나 재발 및 합병증은 보이지 않았다. 결론: 병변내 시도포비어 주입술은 편평상피유두종의 치료에 완전절제술과 병용하여 시도해 볼 수 있는 특별한 합병증이 없는 치료법으로 완전절제술 후에도 재발을 보이는 편평상피유두종 환자에서 부가적인 치료로 고려할 만한 시술이다.
Abstract
Purpose: To report a case of treating a patient with intralesional cidofovir injection who had frequently recurring lacrimal sac squamouspapilloma after several excision surgeries. Case summary: A 59-year-old man who had mass excision surgery at a different clinic nine months previously, visited our clinicto treat a recurring erythematous protruding mass near his left medial canthus that developed two months prior. Orbit CT showeda 15 x 25 mm-sized large mass located on the lacrimal sac adherent to medial orbital wall. An excision biopsy was performed andthe histopathologic examination showed typical findings of squamous papilloma. Because the tumor recurred six months afterthe second surgery, we decided to perform adjuvant therapy using the antiviral agent cidofovir. The patient was treated with a 5mg/mL intralesional cidofovir injection every three weeks. A transient recurrence presented on the upper lid at the third intralesionalcidofovir injection site two months after the surgery, but the recurrent lesion improved after repeated injections. During the12 months of follow-up, there were no complications and no evidence of recurrence. Conclusions: Intralesional cidofovir injections can be a safe and effective treatment for the management of recurrent squamouspapilloma of the orbit, especially after total excision.
