목적: Epstein-Barr 바이러스 감염으로 인한 급성 눈물샘염을 경험하였기에 보고하고자 한다. 증례요약: 19세 여환이 2주간의 양측 눈꺼풀의 통증, 발적 및 부종을 주소로 내원하였다. 2주 전 콧물, 기침, 가래 등의 감기 증상및 발열이 있었다고 하며 양측 경부림프절의 비대소견이 관찰되었고, 안와 전산화단층촬영에서 양측 눈물샘의 염증성 비대소견을보였다. 혈액검사상 ESR 및 간기능 수치가 상승해 있었으며, 특수 항체검사상 Epstein-Barr 바이러스 IgM, IgG양성 반응을 보여 임상적으로 Epstein-Barr 바이러스 감염에 의한 급성 눈물샘염으로 진단하였다. 대증치료로 눈물샘 염증은 소실되었으며 3개월간의 경과관찰 기간 동안 재발소견은 없었다. 결론: 양측성의 급성 눈물샘비대 소견이 있으면서 감기 증상 및 발열 등의 전신증상, 림프절 비대 및 혈액검사상 Epstein-Barr 바이러스 항체 양성반응이 있을 경우, Epstein-Barr 바이러스 감염에 의한 급성 눈물샘염으로 진단이 가능하며 대증적인 치료로 호전이될 수 있다.
Abstract
Purpose: To report a case of Epstein-Barr virus-related dacryoadenitis. Case summary: A 19-year-old female presented with pain, swelling and redness of both upper eyelids. She experienced rhinorrhea,cough and fever for the previous two weeks. Physical exam showed bilateral cervical lymph node enlargement, and orbitalcomputed tomography (CT) revealed diffuse enlargement and inflammation of both lacrimal glands. Serological testing revealedelevated titers for Epstein-Barr virus nuclear antigen immunoglobulin G (IgG) and Epstein-Barr viral capsid antigens IgG and immunoglobulinM (IgM). Based on these results, clinical diagnosis of Epstein-Barr virus-related acute dacryoadenitis was made. The inflammation subsided after conservative management. Relapse was not observed during the three-month follow-up period. Conclusions: Based on acute bilateral lacrimal gland enlargement, systemic symptoms such as fever, rhinorrhea, cough, lymphadenopathyand serologically-positive results for Epstein-Barr virus, Epstein-Barr virus-related acute dacryoadenitis can beclinically diagnosed and treated with conservative therapy.
