연령관련황반변성의 혈관내피성장인자억제제 주사치료 후 발생한망막색소상피파열의 장기 경과관찰

Long-Term Observation of Retinal Pigment Epithelial Tear after Anti-VEGF Treatment for Age-Related Macular Degeneration

초록

목적: 연령관련황반변성에서 유리체강 내 혈관내피성장인자억제제 주입술 후 발생한 망막색소상피파열의 1년 이상 장기 경과를 관찰하여 보고하고자 한다. 대상과 방법: 2009년 2월부터 2013년 6월까지 4년 4개월 동안, 연령관련황반변성에 대해 라니비주맙이나 베바시주맙을 유리체강내 주사한 후 망막색소상피 파열이 발생한 환자 중 12개월 이상 경과관찰이 가능했던 13명 13안을 대상으로, 파열 후 경과관찰에서치료 지속 여부, 시력변화, 빛간섭단층촬영에서 낭포황반부종의 변화, 중심황반두께의 변화, 파열의 중심와 오목 침범 여부에 따른시력예후 등을 후향적으로 조사하였다. 결과: 파열 후 치료를 지속한 환자 수는 13명 중 12명이었고 파열 후 평균 주사 횟수는 6.08 ± 5.18회였다. 파열 직후 평균 logMAR시력은 1.65 ± 0.8, 최종시력은1.82 ± 0.88이었다. 빛간섭단층촬영상 황반부종이 있었던 9안은 계속 주사하여 4안에서 황반부종이호전되었다. 빛간섭단층촬영에서 파열부위가 중심와 오목을 침범한 눈에서 최종 평균 logMAR 시력은 2.17 ± 0.49로, 침범하지 않은눈에서의 1.51 ± 1.06보다 시력예후가 나빴으나 통계적 의의는 없었다(p=0.295). 결론: 망막색소상피파열이 발생한 후에도 치료를 지속하면 파열직후보다 시력은 호전되지 않았으나 해부학적 지표들은 호전되는 경과를 보였다. 중심와 오목이 파열부위에 포함된 경우는 포함되지 않은 경우보다 시력은 좋지 않았으나 통계적 의미는 없었다.

Abstract

Purpose: To evaluate the long-term clinical outcomes of retinal pigment epithelium (RPE) tears after intravitreal injection of anti-vascular endothelial growth factor (VEGF) agent for the treatment of neovascular age-related macular degeneration (AMD). Methods: The authors performed a retrospective chart review of 13 eyes of 13 patients who developed RPE tears after intravitrealanti-VEGF injection between February 2009 and June 2013. We investigated continuation of the treatment after tear, visualacuity, presence of cystoid macular edema, and central macular thickness (CMT) using optical coherence tomography (OCT)before and after treatment and visual outcomes depending on foveal sparing. Results: After RPE tear, 12 of 13 patients continued injection of an anti-VEGF agent. The average number of injections was 6.08± 5.18. Mean visual acuity immediately after tear was 1.65 ± 0.8 log MAR, and that at the last visit was 1.82 ± 0.88 log MAR. Nineeyes with macular edema in OCT continued receiving injection, and improvement of macular edema was observed in four eyesat the final visit. The final visual acuity of patients with foveal involvement was 2.17 ± 0.49 log MAR, which was worse than the1.51 ± 1.06 log MAR in patients without foveal involvement, although the difference was not significant (p = 0.295). Conclusions: When anti-VEGF injections were continued after RPE tear, no improvement in visual acuity was observed, althoughbetter anatomical outcomes did result. Patients with foveal involvement had worse visual acuity than patients without fovealinvolvement, but the difference was not significant.
