목적: 수술적 합병증과 관련 없이 발생하는 자발성 맥락막상강 대량 출혈은 매우 드문 것으로 알려졌다. 저자들은 과도한 음주력을가진 중년 여성에서 발생한 자발성 맥락막상강 대량 출혈을 경험하여 이에 대해 보고하고자 한다. 증례요약: 과잉 알코올 섭취력을 가진 48세 여자환자가 우안의 갑작스런 시력저하를 주소로 내원하였다. 안저 검사 및 초음파 검사상우안의 맥락막 상강 출혈을 동반한 전체망막박리와 함께 접촉맥락막 소견이 관찰되었다. 전신적 검사상 알코올 유도성 간병증 및당뇨가 진단되었다. 환자는 공막편을 통한 맥락막상강 출혈의 외부 배액 및 유리체 절제술, 그리고 안구내 실리콘 기름 주입술을 시행받았다. 결론: 저자들은 과잉 알코올 섭취 환자에서 발생한 자발성 맥락막상강 대량 출혈을 경험하여 이를 국내 최초로 보고하는 바이다.
Abstract
Purpose: To report a case of massive spontaneous suprachoroidal hemorrhage in a middle-aged female with heavy alcoholconsumption. Case summary: A 48-year-old female with a history of heavy alcohol consumption developed a massive spontaneous suprachoroidalhemorrhage in the right eye. Fundus examination and B-scan ultrasonography of the right eye revealed a hemorrhagicchoroidal detachment with kissing choroidal sign. On systemic evaluation, she was diagnosed with alcohol-induced hepatitis,diabetes, and diabetic nephropathy. We performed trans-scleral drainage of the suprachoroidal hemorrhage, trans pars plana vitrectomyand silicone oil injection in her right eye. Conclusions: To our knowledge, this is the first case report of massive spontaneous suprachoroidal hemorrhage in a patient withheavy alcohol intake.
