올가미를 이용하여 제거한 말단 회장의 화농성 육아종 1예

화농성 육아종은 모세혈관 크기 혈관들의 분엽상 증식, 간질의 부종 및 염증세포 침윤 등의 조직병리소견을 특징으로 하는 혈관종의 일종으로, 표면에 미란이나 궤양을 흔히 동반하는 용종형태를 보이며 피부나 점막표면에 호발한다. 구강을 제외한 위장관에서의 발생은 극히 드물어 전 세계적으로 약 26예가 보고되었는데, 80% 이상이 일본, 한국 등 동아시아 지역에서 보고되었다. 식도부터 항문까지 전체 위장관에 걸쳐 발생하며 식도와 소장에 가장 흔하다. 혈변 혹은 흑색변이 가장 흔한 증상이며 대개 유경성 혹은 아유경성 형태를 보이고 크기는 0.3～3.0 cm까지 다양하다. 저자 등은 빈혈 소견을 보인 57세 남자에서 대장내시경 검사를 시행하여 말단 회장에서 표면에 미란, 발적, 백태가 있는 유경성 종괴를 발견하였고, 올가미를 이용해 절제한 결과 화농성 육아종으로 진단된 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
Abstract
Pyogenic granuloma (PG) is a polypoid form of capillary hemangioma occurring on the skin and mucosal surfaces. The well-established lesion is a rapidly growing, polypoid, friable, purple-red mass that bleeds easily and frequently ulcerates. Microscopically, it is composed of small blood vessels arranged in a distinctive lobular pattern with or without ulceration of the surfaces. Except for the oral cavity, this lesion is extremely rare in the gastrointestinal tract and up to the present, about 26 cases of gastrointestinal PG have been reported. Of them, 21 cases were reported from East Asia, Japan, Korea, and Taipei. The most common presenting sign of gastrointestinal PG was tarry stool and anemia. In most cases, lesion was solitary, and revealed a protruded and/or pedunculated growth pattern with superficial erosions. We have recently experienced a case of pyogenic granuloma in the terminal ileum and successfully treated by endoscopic snare polypectomy. (Korean J Gastrointest Endosc 2002;24:176-180)
