내시경 췌관괄약근 절개술 339예의 임상적 적용 및 안전성

초록

목적: 내시경 췌관괄약근 절개술(endoscopic pancreatic sphincterotomy, EPST)과 췌관배액술이 만성췌장염을 비롯한 췌장질환 치료의 새로운 방법으로 시도되고 있으나, 그 적응증과 안전성에 대한 자료의 부족으로 내시경 담관괄약근 절개술(endoscopic biliary sphincterotomy, EST)에 비해 널리 사용되지는 못하고 있다. 또한 EPST의 합병증은 3.2～15.7%로 다양하게 보고되고 있다. 본 연구에서는 EPST의 적응증, 합병증 및 안전성에 대해 알아보고자 하였다. 대상 및 방법: 2000년 1월부터 2004년 4월까지 서울아산병원에서 EPST를 시행한 339명의 환자를 대상으로 하였다. 결과: 전체 339명의 환자 중 EPST 단독으로 시행한 경우가 252예(74.3%), EST를 같이 시행한 경우가 87예(25.7%)였다. 적응증은 췌관 결석 142예, 췌관 협착 97예, 췌장 가성낭종 35예, 완전형 분할췌 19예, 불완전형 분할췌 13예, 췌장암 9예, 누공 17예, intraductal papillary mucinous neoplasm (IPMN) 9예, 오디괄약근 기능이상이 8예였다. 합병증은 총 37예(10.91%)에서 발생하였고, 의미있는 출혈 6예(1.8%), 췌장염 15예(4.4%), 천공 3예(0.9%), 협착 9예(2.7%), 감염 4예(1.2%)였다. EST를 함께 시행한 군과 EPST 단독으로 시행한 군 간의 합병증 빈도의 차이는 없었다. 주췌관 EPST는 287예, 부췌관 EPST는 46예, 주췌관과 부췌관 모두 EPST를 시행한 경우는 6예였고 각 군간의 합병증의 빈도 차이는 통계학적 의미가 없었다. 췌관배액술을 함께 시행한 경우나 만성 췌장염 환자에서 EPST를 시행한 경우 췌장염의 빈도가 낮았지만 통계학적인 의미는 없었다. 합병증이 발생한 경우 내과적 치료로 모두 호전되었으며 합병증으로 인한 사망은 없었다. 결론: 내시경 췌관괄약근 절개술은 다양한 췌장질환의 내시경 치료에 이용될 수 있으며 기존에 보고된 EST나 EPST의 합병증 빈도와 큰 차이가 없이 안전하게 사용될 수 있으리라 생각한다.

Abstract

Background/Aims: Endoscopic pancreatic sphincterotomy (EPST) has been performed more frequently in recent years. However, it is less widely practiced than biliary sphincterotomy due to lack of firm scientific data regarding its indication and safety. The aims of this study are to evaluate EPST with regard to indications, complications, and safety. Methods: We retrospectively reviewed and analyzed the results of EPST performed in three hundred thirty nine patients from January 2000 to April 2004. Results: Complications occurred in 37 patients (10.7%) which included pancreatitis, hemorrhage, perforation, cholangitis, sepsis, and stenosis of sphincterotomy site. They were successfully managed by medical treatment. No mortalities were reported. Conclusions: EPST is a relatively safe procedure in various pancreatic diseases. Incidence of long-term complications awaits further investigations. EPST enlarges our endotherapeutic armamentarium and deserves additional evaluation.
