자가면역성 췌장염 진단에서의 내시경 검사의 역할

초록

자가면역성 췌장염은 스테로이드에 대한 좋은 치료반응을 특징으로 하는 특이한 형태의 만성췌장염으로 진단 시에 감별해야 할 가장 중요한 질병은 담관 및 췌장의 악성질환이다. 전형적인 자가면역성 췌장염의 영상소견은 CT에서 췌장의 전반적인 종대가 관찰되며 ERCP에서는 주췌관이 전반적으로 가늘고 좁아져 있는 것이 특징이다. 비전형적인 자가면역성 췌장염의 영상소견으로는 국소종대, 췌장종괴, 주췌관의 국소협착 등이 있는데 이 경우 영상소견만으로는 췌장암과의 감별이 어려우므로 초음파내시경 검사 및 조직검사가 필요하다. 담관침범에 의한 폐쇄성 황달이 동반된 경우는 담관암과의 감별을 위해 ERCP검사 시에 담관 협착부위에 대하여 쇄자세포진 및 겸자생검을 시행하는 것이 추천된다. 저자들은 이 종설에서 자가면역성 췌장염의 진단에 대하여 내시경 검사, 특히 ERCP와 초음파내시경검사의 역할에 중점을 두어 기술하였다.
