Backgroud : Antiphospholipid antibody syndrome(APS) is associated with a hypercoagulable state characterized by thrombocytopenia, fetal loss and stroke. Although optic neuropathy is another manifectation in APS, its MRI finding has been rarely reported. Case Report : A 62-year-old woman visited our hospital because of progressive altitudinal visual defect over 1 week. She had a history of having left thalamic lesions twice(4 years and 2 years before this event). Positive lupus anticoagulant and antinuclear antibody(1:80) tests were detected, and anticoagulation therapy was started. On orbital MRI, a focal enhancing lesion in the optic nerve was detected. She was additionally treated with methylprednisone pulse therapy and her visual defect was improved. Conclusion : We report a first case of MRI proven ischemic optic neuropathy in a patient with APS. Korean Journal of Stroke 2001;3(2): 185~187
