초록
Department of Neurology, College of Medicine, Seoul National University, Seoul National University Bundang Hospital A 47-year-old male developed sudden loss of consciousness about 7 hours after bilateral tibio-fibular shaft open fracture without orthopedic manipulation. Brain CT showed no abnormality. Emergency brain T2-weighted MRI demonstrated scattered small lesions on periventricular white matter, thalamus, basal ganglia and cerebellar hemisphere with high signals. Diffusion weighted images revealed bright spots on a dark background (“starfield” pattern). The patient was comatose for about a month. A follow-up MRI showed slightly denser signals than what was shown in previous images. We suggest that diffusion weighted image is valuable for immediate detecting of cerebral fat embolism.
